About this file
This is a Cost Sheet attachment for the State of Utah Division of Purchasing's RFP MK26-32 ORS Genetic Testing, conducted by the Department of Health and Human Services. The procurement seeks to establish a single contract award for laboratory and genetic testing services, with an anticipated contract term of five years. Vendors must guarantee their pricing for a minimum of one year during the contract period.
Pricing for this procurement is structured as a firm, fixed price per draw, with the price point inclusive of all associated costs including fees, insurance, licenses, fuel, wages, salaries, leases, rent, bonding, taxes, vehicle maintenance, travel, expert testimony, and all other expenses. The firm price per draw should account for the completion of draws for all individuals necessary to complete paternity testing determinations. Evaluation of pricing will utilize a cost point allocation method whereby the vendor submitting the lowest price per draw receives the full 514.3 cost points available. All other vendors will receive cost points calculated on a proportional basis using the formula: Cost Points x (Lowest Proposed Price/Proposed Price), ensuring that pricing is a significant factor in the award determination process.
