About this file

This is an RFP evaluation scoresheet for SOLICITATION #MK26-32 ORS Genetic Testing issued by the State of Utah Division of Purchasing, with the Department of Health and Human Services as the conducting procurement unit. The RFP seeks to contract with a single responsive and responsible vendor to provide laboratory and genetic testing services. The contract term is five years, and vendors must guarantee pricing for a minimum of one year.

The evaluation scoresheet establishes a weighted scoring system totaling 1,714.3 possible points, comprised of 70% technical evaluation (1,200 points) and 30% cost evaluation (514.3 points). Technical scoring is based on six criteria: Service Areas and Collection Site Logistics (300 points), Testing Procedures, Reporting, and Record Retention (250 points), Experience (250 points), Training (200 points), Quality Assurance and Customer Service Requirements (100 points), and Data Transmission and sFTP Configuration (100 points). A minimum technical score threshold of 840 points (70% of technical evaluation) is required for proposal advancement. Ten mandatory requirements must be met for consideration, including proof of applicable licensing and accreditation by AABB or similar federal designee for a minimum five years, submission of a W-9 form, five years of government child support enforcement agency experience, ability to transmit results via sFTP, and submission of both a Technical Proposal and Cost Sheet. Cost points are calculated using a formula where the lowest proposed price receives 100% of cost points, with all other proposals receiving proportional points based on their price differential relative to the lowest bid.
