About this file

This is a Scope of Work attachment for a genetic testing contract between the State of Utah Department of Health and Human Services (DHHS) Office of Recovery Services (ORS) and a contracted laboratory provider. The contract establishes comprehensive requirements for providing laboratory and genetic testing services to determine parentage for child support cases across Utah. The contractor must maintain certification through either the Association for the Advancement for Blood and Biotherapies (AABB) or the American Society for Histocompatibility and Immunogenetics (ASHI) and employ certified phlebotomists meeting all Utah legal requirements. Sample collection services are required at four primary ORS office locations in Ogden, Provo, Taylorsville, and St. George, with additional collection services available statewide, nationally, and internationally, including at correctional facilities. Testing must utilize polymerase chain reaction methodology with a minimum of 15 DNA systems per case, achieve a 99% probability of paternity for inclusionary cases, and comply with strict chain-of-custody requirements outlined in Utah Code 81-5-504. Test results must be transmitted as PDF documents via secure file transfer protocol to the DHHS sFTP server within 24 hours of completion, accompanied by professional laboratory reports signed and notarized by qualified experts holding Ph.D.s in biological or biomedical sciences. The contract term is five years with vendor pricing guaranteed for one year.

The contractor is responsible for all costs associated with genetic testing procedures, including phlebotomy supplies, courier services, and re-collections due to specimen viability issues. Monthly invoices must be submitted within 10 to 15 days following the end of each service month and organized by ORS office location, with each invoice containing specific participant information, case numbers, sample receipt dates, and per-sample pricing. The contractor must maintain 100% compliance with chain-of-custody documentation, achieve over 98% first-time sample collection success rates, deliver 90% of results within 24 hours of completion, and maintain a 95% on-time delivery rate for all contractual deadlines. The contractor shall provide no-cost court support services including depositions, interrogatory responses, affidavits, and testimony, operate a toll-free customer support line, and provide a secure web-based testing status review system. Monthly and quarterly performance reports are required, tracking operational metrics, sample collection statistics, turnaround times, billing accuracy, and compliance activities, with all records maintained according to AABB, ASHI, and applicable state and federal standards.
