About this file
This document is a Cost Sheet Attachment for a Request for Proposal (RFP) issued by the State of Utah Division of Purchasing, specifically solicitation #SB25-13 for the Publication of Utah Code Books. The RFP seeks to establish a contract for legal research, citation services, printing, and delivery of various publications for State of Utah Eligible Users, with potential for multiple awards. The contract will have a 5-year term, with vendors required to guarantee pricing for one year. The specific scenario detailed in the document involves a sample order of 300 Utah Annotated Code Books (Volume 1 only), with precise specifications including 800-page volumes, specific paper characteristics, and a hardbound blue cover with gold ink.
The cost sheet requires bidders to complete detailed pricing information, including per-page and per-volume printing costs, shipping costs, update/supplement pricing, and proof costs. Vendors must also propose bulk discount tiers to be considered for the contract. The evaluation methodology is points-based, with a total of 1000 points available, requiring proposals to achieve 700+ points to be awarded a contract. The evaluation process includes technical and cost criteria, with 300 points allocated to cost evaluation using a formula that compares proposed prices. Importantly, failure to complete all yellow-highlighted areas in the cost sheet will result in proposal disqualification, and offering some form of bulk discount is mandatory for consideration.
