About this file

This document is a Proposer's General Questionnaire for a Third Party Leave Administrator contract issued by the University of Texas Health Science Center (UT Health) in San Antonio, Texas. The document provides a comprehensive template for potential vendors to complete, detailing requirements for proposing services. The questionnaire covers multiple sections including Proposer Profile, Approach to Work, General Requirements, Service Support, Quality Assurance, and Miscellaneous aspects of the proposed contract. Proposers are instructed to provide detailed information about their company, including business history, financial stability, references, service approach, key personnel, and support capabilities.

The document emphasizes that most sections (Approach to Work, General Requirements, Service Support, Quality Assurance, and Miscellaneous) should be clearly presented in the submitted proposal, not within this questionnaire template. Key requirements include providing financial statements, references from at least three organizations, disclosure of any relationships with university employees, cloud computing service details, and potential additional services or benefits. The proposer must also address potential difficulties in performing duties, describe service support philosophy, quality assurance programs, and include a contingency or disaster recovery plan if applicable. The document is structured to comprehensively evaluate potential third-party leave administrators through a standardized, detailed questionnaire process.
