About this file

This document is an Information Security Initial Security Posture Assessment (ISPA) form from UT Health San Antonio for a Third Party Leave Administrator contract. The form is designed to comprehensively evaluate a potential vendor's readiness to handle sensitive university information, with a focus on privacy, security protocols, and data protection practices. The assessment covers multiple sections including provider information, privacy risk analysis, and security risk analysis for electronic systems, requiring detailed responses about the vendor's information security policies, procedures, and technical safeguards.

The assessment requires vendors to provide extensive details about their information security practices, including workforce composition, subcontractor management, device and system configurations, access controls, encryption methods, breach response protocols, and compliance with regulatory requirements. Key areas of focus include identifying authorized users, protecting sensitive information, managing remote access, implementing password policies, securing physical and electronic systems, monitoring for potential security incidents, and ensuring proper disposal of sensitive data. Vendors must demonstrate comprehensive policies and technical controls to protect university-sensitive information across various domains, with specific attention to HIPAA compliance, data transmission security, and incident response capabilities.
