About this file

This is a Critical Finding Inspection Report for a bridge structure conducted by the USDA Forest Service. The W Fork of San Juan River Bridge (Structure #021306000001024) located on Route 648 in Colorado at mile point 2.50 sustained severe damage from an October 2025 flooding event, resulting in immediate closure as of October 23, 2025. The inspection, conducted on October 27, 2025 by Team Lead Jason Hefter and team members Tucker Bowman and Benjamin Glenn, classified the bridge as Fair (F) condition with a lowest component rating of 6 (Satisfactory).

The critical findings document damage from the monsoon flooding event requiring immediate corrective action. The right abutment experienced severe scour undermining up to 4.5 feet deep (measured toe to stream bed bottom) and 4 feet under center, compromising structural support. The channel condition rating is 2 (Critical), with significant deterioration of upstream and downstream banks on both sides requiring bank revetment and stream key piece material to realign the channel to center. Additional deficiencies include a damaged bridge railing transition rated 4 (Poor), scour hole requiring approximately 16 cubic yards of engineered structural fill, and missing channel protection requiring approximately 145 cubic yards of replacement material on the river right side. Future actions specified include underpinning or replacing the abutment and implementing scour countermeasures and stream training measures. The bridge is owned and maintained by the U.S. Forest Service, and repair work will be essential to return the structure to service.

B.C.04 Culvert Condition Rating (N - NOT APPLICABLE - Component does not exist.)

B.C.06 Bridge Railing Transitions Condition Rating (4 - POOR - Widespread moderate or isolated major defects;

strength and/or performance of the component is affected. )

Minor increases in previously damaged transition due to overtopping of bridge.

B.C.09 Channel Condition Rating (2 - CRITICAL - Major defects. Bridge or approach roadway is severely threatened.

Condition typically necessitates frequent monitoring, significant load restrictions, and/or corrective actions in order to keep the bridge open.)

Channel realigned to attack right abutment. Significant deterioration of upstream and downstream banks on both river left and river right. Bank revetment needed on all sides to prevent further erosion. Stream key piece material needed to realign stream to center channel. Large debris jam present on left abutment.

B.C.11 Scour Condition Rating (1 - Bridge is closed to traffic due to scour condition. Channel rehabilitation may return the bridge to service.)

Right abutment severely undermined - max 4.5 feet deep (measured toe to stream bed bottom) and 4 feet under center (measured toe towards back of abutment) .
