About this file
This is a Critical Finding Inspection Report from the USDA Forest Service documenting structural deficiencies identified during an inspection of the W Fork of San Juan River bridge on Route 648 in Colorado (Structure Number 021306000001024).
The bridge, located 13.7 miles northeast of Pagosa Springs at mile point 2.50, was inspected on April 8, 2026, by Team Lead Benjamin Glenn and reviewed by Jason Hefter. The overall structure condition is rated as Fair (F). A critical finding was initially documented on October 22, 2025, following the 2025 Colorado October 10-13 Four Corners Monsoon Storms event, with the finding status remaining RO-Active as of the April 2026 inspection. The critical issue involves abutment undermining on the right abutment caused by scour from flooding and rain events. The CF Criterion requires immediate correction of scour or hydraulic problems. Initial actions included closing the bridge and placing temporary riprap to divert flow toward the channel center on October 22, 2025. As of April 6, 2026, the bridge remained closed. Riprap has been installed under and around the entire right abutment including wing walls; however, the scour hole persists and has grown significantly (50%) at one measurement location. Future corrective actions required include underpinning or replacing the abutment and adding scour countermeasures and stream training measures. Condition summary ratings are: B.C.01 - 6, B.C.02 - 7, B.C.03 - 6, B.C.04 - N, B.C.09 - 2, and B.C.11 - 1. The report includes extensive photographic documentation of scour conditions, riprap installation, abutment damage, guard rail damage, and deck-abutment joint gaps.
