Respondents indicate whether performance involved Security Officer Services, Program Management, Configuration Management, Quality Control Program, Security Officer Training, or other areas. Part Three contains 16 questions in the Program Management section covering contractor success in providing trained personnel, managing project phases, demonstrating initiative, quality control effectiveness, transition schedules, data reporting timeliness, key personnel competence and communication, corporate management support, employee turnover minimization, and overall contract management. Additional yes/no questions address whether major contract requirements were terminated, cure or show cause notices were issued, or options were not exercised due to poor performance. Part B addresses training requirements with five questions on training development, facility adequacy, and satisfaction of training needs. Final sections request optional referrals, an overall recommendation for contract award, remarks, and respondent information to be kept confidential, including name, position, organization, telephone number, length of program involvement, and completion date.
