About this file
This document is a Past Experience Questionnaire template for federal contract solicitation SIR No. 693KA8-26-R-00003. Offerors must complete this form to demonstrate past experience and performance on contracts similar in scope and complexity to the requirements outlined in the solicitation's Statement of Work (Section C). The questionnaire serves as supporting documentation for the FAA's evaluation of proposals, and each customer cited must also complete the accompanying Past Performance Questionnaire (Attachment L.2) to be returned to the designated FAA office listed in Section L.5.
The questionnaire requires offerors to provide specific contract information including contracting agency, customer points of contact (Program Manager and Contracting Officer), contract number, contract type, period of performance, original and current contract values, and a detailed description of work effort with clear designation of whether the offeror served as prime contractor or subcontractor. For prime contractors, the form requires identification of integration responsibilities. Offerors must also explain relevant experience by selecting applicable evaluation factors from Section M.5 and demonstrating how past contracts apply to the current solicitation's requirements, with particular emphasis on five specific content areas: transition of services, initial and recurring training, acquiring and renewing individual and corporate certifications and qualifications, record keeping, and quality control. If subcontractors, teaming partners, or joint venture members are involved, their relationships and work performed must be specifically described. Each Past Experience Questionnaire is limited to three pages, and offerors may submit a maximum of three completed questionnaires.
