Remarks

39-Prestressed Slab SU 1 ENV 3 The slabs are 21 inches deep.

1081-Soffit Spalls/Delams/Patches Slab 5 exhibits a shallow spall between the 1st and 2nd utility attachment.

511-AC Wearing Surface The asphalt wearing surface exhibits 1/4-inch wide longitudinal construction joint in the center of the southbound lane. A semi-straight 1/4-inch wide crack has formed in the center of the northbound lane.

215-Re Conc Abutment SU 1 ENV 3 The original abutments were left in place and serve as end slope protection/retaining walls. These original abutments exhibit severe undermining due to channel scour. The east side of abutment 2 end slope exhibits an 8-foot diameter collapsed area upstream from the old abutment, but does not pose a scour risk at this time.

The abutment 1 slope behind the original abutment exhibits sloughing and collapsed riprap.

306-Other Joint SU 1 ENV 3 2310-Leakage Both approach pavements exhibit transverse cracks up to 1/4-inch wide.

330-Metal Bridge Railing SU 1 ENV 3 The northeast and northwest end treatments exhibit minor impact but are still substantially effective. There is vegetation overgrowing the bridge rails.

1000-Corrosion The top of the bridge rails exhibit light blush rusting.

515-Steel Protective Coating The galvanizing coating on the W-beam bridge rail is thinning.

990-Miscellaneous Elements SU 1 ENV 3 Rated for the 4-inch diameter PVC pipe attached to the upstream bridge rail and the 12-inch diameter ductile iron pipe suspended from slab 5.

999-Roadway Impact SU 1 ENV 3 Both approach pavements exhibit spalls and upheaving up to 3/4-inch high in the shoulder areas along the bridge transitions. The adjacent bridge deck AC is also heaving, further contributing to the rough transition.
