Remarks

12-Re Concrete Deck SU 1 ENV 3 There is an accumulation of dirt and debris on the deck.

1081-Soffit Spalls/Delams/Patches The deck soffit overhangs exhibit several small open incipient popouts with exposed rebar.

1090-Exposed Rebar The left curb at abutment 1 exhibits impact related spalling to the inboard face with exposed rebar.

1120-Efflorescence/Rust Staining Span 1 deck soffit exhibits a transverse crack with efflorescence in bay 1 near abutment 1.

107-Steel Opn Girder/Beam SU 1 ENV 3 FPD: The welds are continuous around the base of all vertical stiffeners creating a fatigue prone detail (category E) at the bottom flange.

The span 3 beam ends are in contact with the abutment 4 backwall. Beams exhibit no signs of distress at this time.

1000-Corrosion The paint system on the girders exhibits isolated areas of failure resulting in light surface corrosion.

518-Steel Paint The paint system on the girders exhibits isolated areas of failure along the bottom flange but the paint system is substantially effective.

205-Re Conc Column SU 1 ENV 3 Previous scour at Bent 3, Column 1 has been repaired with crushed rock.

215-Re Conc Abutment SU 1 ENV 3 Moderate moss growth on both abutments.

All of the wingwalls are rotated outward leaving a gap between the wingwall and the abutment: northwest 1/2-inch, southeast 1 1/2-inch and southwest 5/8-inch. The northeast wingwall exhibits a fractured piece of concrete at the top making rotation measurements difficult to replicate. Measurement for the northeast is measured from face of cheek wall to top of wingwall construction joint and measures 11.25 inch.

The northwest wingwall is broken at the top. The northeast wingwall has fractured and the post-tension restraint plate is no longer seated flush.

1080-Delamination/Spall/Patched Area Abutment 4 is delaminated at the northeast wingwall.

1130-Cracking (RC and Other) Both abutments exhibit full height vertical cracks up to 0.016-inch wide near the centerline and beneath the bearings.

234-Re Conc Pier Cap SU 1 ENV 3 Bent 2, north face has 4 rebars protruding out approximately 1 foot at the west end of the cap. Bent 3, south face has 1 rebar protruding out up to 1 foot at the east end of the cap.

1090-Exposed Rebar Bent 3, east cantilever underside exhibits 3 exposed longitudinal reinforcing bars. Exposed reinforcing steel can be attributed to insufficient concrete cover and does not exhibit any measurable section loss.

306-Other Joint SU 1 ENV 3 The joints over bents 2 and 3 are covered by chip seal.

2310-Leakage The chip seal exhibits an 1/8-inch wide crack at the joint over bent 2. Both approach pavements exhibit a transverse crack up to 1/2-inch wide at the bridge transitions due to settlement of the approach roadway.

311-Moveable Bearing SU 1 ENV 3 The bearing grout pads of girders 1, 2 and 3 at bent 3 exhibit construction related voiding beneath their masonry plates.

Moveable bearings exhibit fretting corrosion indicating movement. Bearings at bent 3 are near vertical at 50 degrees. Pack rust is forming under the masonry plate for beam 2 span 2 at bent 3.

2210-Movement The bearings at abutment 4 are at limits of expansion at 50 degrees. The girders are impacting the backwall of the abutment. Movement of the abutment is the suspected cause. Span 3 fixed bearings at bent 3 show no signs of distress.

518-Steel Paint Minor surface corrosion has initiated at the rocker to sole plate interfaces.

313-Fixed Bearing SU 1 ENV 3 1000-Corrosion Minor pack rust between sole plates and between grout pad and masonry plates at bent 3.

518-Steel Paint Paint system on fixed bearings exhibits isolated failures, typically along sole plate interface.

330-Metal Bridge Railing SU 1 ENV 3 The right bridge railing exhibits scraps and dents due to minor collision damage throughout. The southwest end treatment exhibits minor impact damage but is still functional.

1000-Corrosion Thinning of the bridge railing galvanizations has resulted in blush rusting forming on the tops of the rail panels.

1020-Connection The first rail post on the left bridge railing is disconnected from the curb and rail.

390-Ptd Bridge Element SU 1 ENV 3 The paint on the superstructure is in good condition. Minor surface corrosion has initiated along the edges of the bottom flange and at the bearing rocker to sole plate interfaces.

999-Roadway Impact SU 1 ENV 3 South approach pavements exhibit settlement up to 1-inch.

North approach roadway exhibits 2-inches of settlement.

Notes Inspection Notes

[WTS 11/4/2020]: 6 year FP/Snooper inspection frequency at inspector discretion due to axial loads in span 3 beams. Use snooper to stringline bents for plumb.
