Remarks

16-Re Conc Top Flange SU 1 ENV 3 1120-Efflorescence/Rust Staining The deck soffit exhibits two diagonal cracks, one with minor rust staining in bay 1, near abutment 2.

511-AC Wearing Surface Deck top received a fresh lift of chip seal in 2020.

110-Re Conc Opn Girder/Beam SU 1 ENV 3 1130-Cracking (RC and Other) The girders exhibit hairline width shear cracks near abutment 1.

215-Re Conc Abutment SU 1 ENV 3 Abutment and wingwall footings are monolithic. A construction joint exists between the wingwalls and abutments; therefore, abutment lengths do not include wingwalls.

1120-Efflorescence/Rust Staining Abutment 1 exhibits a horizontal crack with 1-foot of efflorescence between beams 3 and 4. Abutment 2 exhibits horizontal cracks with efflorescence. Both wingwalls of abutment 2 exhibit 1/64-inch wide cracks with efflorescence.

1130-Cracking (RC and Other) Abutment 2 exhibits horizontal cracks up to 1/64-inch wide.

1190-Abrasion(PSC/RC) The bottom 3-feet of both abutments and the exposed footing at abutment 1 exhibit moderate abrasion/scale damage.

221-RC Spread Ftg SU 1 ENV 3 6000-Scour Abutment 1 footing has silted in and only the top of the footing is exposed for 5-feet at the north end.

Abutment 2 and upstream (SE) wingwall footing is exposed for 45 feet in length up to 16-inches vertically.

Large diameter riprap is in place at the upstream end of the SE wingwall footing.

306-Other Joint SU 1 ENV 3 2310-Leakage The west transition asphalt concrete exhibits a 1/4-inch wide crack in the eastbound lanes due to settlement. The asphalt concrete over the east transition is cracked 1/4-inch wide full width.

330-Metal Bridge Railing SU 1 ENV 3 Three rail panels of the southwest approach guardrail exhibit moderate impact damage.

1000-Corrosion

Thinning of the bridge railing galvanization has resulted in blush rusting forming on the rail panels.

515-Steel Protective Coating The galvanizing coating on the W-beam bridge rail is thinning.

999-Roadway Impact SU 1 ENV 3 The west approach roadway exhibits 1-inch of settlement in the eastbound lane.

Notes Inspection Notes

Need a 20-foot ladder to access concrete beams. Streambed was dry at the time of the inspection. Best access to underside of structure along gully behind the southwest wingwall. [WTS - 11/5/2020]

Bridge Notes

The bridge was inventoried west to east. Detour map added and length changed by Erick Cain 1/12/2017.

Bridge Hydraulics Notes

There is a large accumulation of drift, inculuding several large trees approximately 50-feet upstream of the bridge, altering the channel flow pattern at the bridge. Substantial aggregation in the center of the channel forces the normal flow at and against abutment 1.
