About this file
This is a Questions and Answers Log for an Invitation for Bid (IFB #6100065715) issued by the Pennsylvania Department of Human Services (DHS) for supplemental medical supplies and equipment to be provided to eleven state-operated facilities across Pennsylvania. The procurement is divided into two separate lots—Lot 1 for medical supplies and Lot 2 for medical equipment—allowing bidders to submit proposals for either or both lots. The specific items required are detailed in Attachment A (Item Specifications), which bidders must reference when preparing their proposals. Mandatory deliverables include completed and signed versions of Attachment A, the Reciprocal Limitations Act Requirements form (GSPUR89), and the Workers Protection and Investment Certification Form. Critical deadlines include April 29, 2026, at 12:00 PM for submission of questions, with answers posted by May 8, 2026, at 12:00 PM, and a final bid due date of May 22, 2026. The contract term commences upon issuance and extends for one year, with options for renewal for up to four additional one-year periods, during which unit prices may be negotiated but shall not exceed a 3 percent increase, plus a potential three-month extension at the Commonwealth's discretion.
The selected contractor must acknowledge receipt of purchase orders within 24 hours, maintain a minimum order threshold of $100.00 for F.O.B. delivered pricing, and bear all shipping and freight costs for qualifying orders. Payment shall be made upon satisfactory delivery and invoice submission on the contractor's letterhead, with reimbursement limited to commodities actually received and accepted by the Commonwealth. The contractor must accept returns of defective or incorrect items within 30 business days at no cost to the Commonwealth without restocking fees. Award will be made to all responsive and responsible bidders per lot, meaning multiple contractors may be selected for each lot. The Manufacturer/Mill Certification form requirement depends on the specific items bid, as different manufacturers have different authorization requirements; bidders must determine whether the form is necessary for their quoted items. The solicitation does not specify particular set-aside designations, no incumbent vendors are identified, and no specific budget ceiling or award value range is provided.

Section Reference Question Answer
Form attachment Manufacturer/Mill Certification
I had a question regarding the MANUFACTURER/MILL CERTIFICATION. I am assuming this form is to assure that resellers have permission to sell products that a certain manufacturer makes? If that is the case, if it is a bid proposal for items from a number of manufacturers, does one need completed by each and every one of those manufacturers? Also, my business is small, so we are often not able to open reseller accounts through the manufacturers themselves, but rather a wholesaler who sells our business a plethora of items from thousands of manufacturers. What would our business do in that instance?
The Manufacturer/Mill Certification form is to be submitted with the invoice for the quoted items shipped to the facility. Therefore, the Attachment A - Item Specifications form lists the items that may require the signed form to certify that your business is an authorized reseller for that item. Whether the form is required depends on the item bid, as different manufacturers have different requirements. Consequently, the bidder must determine whether the form is necessary.
