About this file
This is a Minimum Mandatory Requirements Checklist attachment for a Request for Proposal issued by the Salt Lake City School District (SLCSD) in Utah to procure Pharmacy Benefit Management (PBM) services. The RFP seeks qualified vendors to provide comprehensive PBM services to support the school district's pharmacy benefit program. Vendors must submit technical proposals through the Bonfire/U3P platform and separate price proposals through the RingmasterRx system. Interested vendors are required to contact Tyler Barnes, SLCSD Director of Purchasing, at Tyler.Barnes@slcschools.org to request access to the RingmasterRx system and obtain instructions for completing the price proposal portion of their response. All questions regarding the solicitation must be submitted through the Bonfire system, and only answers issued through Bonfire or authorized addenda constitute the official position of SLCSD. Responses submitted in Bonfire remain secure and invisible to other parties until after the solicitation deadline has passed.
The evaluation of proposals will be conducted in accordance with criteria specified within the RFP document. Vendors are instructed to pay particular attention to RFP language regarding submission requirements and information formatting. Any modifications to the procurement process will be issued only through authorized addenda by the SLCSD Purchasing Department. Vendors who rely on communications or information obtained from sources other than official addenda assume full risk of non-responsiveness determination and potential rejection at SLCSD's sole discretion. The checklist serves as a tool to ensure vendors meet all mandatory requirements before submission.
