About this file
This document is a Conflict of Interest Questionnaire (Form CIQ) used by vendors or persons doing business with a local governmental entity, as outlined in Chapter 176 of the Local Government Code. The form is designed to disclose potential conflicts of interest between a business entity and local government officers, and must be filed with the records administrator within 7 business days of becoming aware of facts requiring the statement.
The questionnaire requires the filer to provide details about their business relationship with the local governmental entity, including whether the local government officer is receiving or likely to receive taxable income from the filer, if the filer is receiving taxable income from the officer, and if the filer is employed by a corporation where the officer serves in a leadership or significant ownership role. Failure to properly complete and file the form can result in a Class C misdemeanor. The document provides a structured approach for transparency in business relationships between vendors and local government entities.
