About this file

This is a cost sheet attachment for a Request for Proposals issued by the State of Utah Division of Purchasing on behalf of the Department of Government Operations Risk Management to solicit third-party claims handling services for property and liability claims. The RFP seeks to award contracts to four qualified vendors capable of providing professional claims management services across the State of Utah. The contract term is five years. Offerors must submit detailed pricing information including all-inclusive travel pricing based on 50 miles of travel that encompasses mileage and drive time, mandatory administration, technology support, or file setup fees with clarification on whether standard line items such as photographs, telephony, postage, and report generation are included or billed separately, and flat-rate fee schedules for ancillary services and file exceptions including erroneous cancellations, re-inspections, and specialty roof or ladder assists.

The cost evaluation component is worth 250 of the 1,000 total available points, with the remaining 750 points allocated to technical criteria evaluation. Vendors must achieve a minimum score of 70 percent or 525 points in the technical evaluation phase to advance to cost evaluation. The lowest-cost proposal will receive 100 percent of available cost points, while remaining vendors will be scored using the formula: Cost Points × (Lowest Proposed Price/Proposed Price). Offerors must complete a sample claim scenario pricing exercise based on 100 claims annually with specified parameters including 70 claims under $10,000, 20 claims in metro areas, 5 involving public adjusters, and average travel distance of 40 miles. Any deviation from the prescribed cost sheet format will result in proposal disqualification, and cost information must be submitted separately from the technical response.
