About this file
This is a Cost Sheet attachment for Solicitation BB26-26 issued by the State of Utah Division of Purchasing and the Department of Health and Human Services for the procurement of Maico EroScan DPOAE hearing screening equipment. The solicitation seeks to establish a single purchase order contract for one year with a vendor capable of supplying five units of Maico EroScan DPOAE hearing screeners operating at 2.0 kHz to 5.0 kHz frequencies, along with ancillary supplies including 10 bags of Sanibel 3-5mm flanged ear tips, five bags of AccuScreen 3.7mm ear tips, 10 bags of AccuScreen 4.5mm ear tips, and three replacement Maico EroScan DPOAE probe cords. The cost proposal must also include estimated annual equipment calibration costs for one unit and shipping expenses. The vendor must guarantee pricing for the one-year PO term, and all costs submitted must be fully loaded and inclusive of labor, materials, travel, and any other expenses necessary to implement and maintain the proposed solution throughout the contract period.
Cost evaluation will be based on the total fully-loaded price submitted on the cost sheet, with technical requirements and cost proposals required to be reflective of one another. The State will award the PO to the responsive and responsible vendor achieving the highest combined score as justified by procurement code, using a cost evaluation formula wherein the lowest proposed price receives 100 percent of available cost points and all other offerors receive a proportional percentage based on how their price compares to the lowest bid. Any costs not explicitly provided on the Cost Schedule will not be allowed into the resulting purchase order; vendors identifying costs that cannot be captured in the current format are instructed to submit modification suggestions through the solicitation's questions and answers board. Deviation from the prescribed cost sheet format may result in disqualification of the offeror's proposal.
