About this file

This is a Technical Criteria attachment for a Request for Statement of Qualifications (RFSQ) issued by the State of Utah Division of Purchasing to establish an open-ended approved vendor list for rangeland services. The solicitation seeks qualified contractors to provide various rangeland treatment services including mastication, lop and scatter, chaining, harrowing, cut and pile, cut and chip, tamarisk and Russian olive removal, and drill seeding. Vendors must submit statements of qualification that are evaluated in three stages: initial review for responsiveness to mandatory minimum requirements, evaluation against specific criteria with a minimum required score of 70 points, and addition to the pre-qualification list for approved vendors. Approval is valid for 12 months, after which vendors must resubmit qualifications for renewal. Contractors already on the current rangeland pre-qualification list are not required to resubmit unless seeking qualification for additional treatment types or acreage categories.

The statement of qualification submission must include a two-page executive summary and a detailed response not exceeding 25 pages, formatted in Times New Roman 12-point font, addressing five weighted evaluation criteria: company and employee experience (35 percent), reference information (25 percent), equipment (15 percent), safety (15 percent), and resumes (10 percent). Vendors must provide company information, documentation of relevant business licenses required in Utah, proof of ability to manage multiple projects simultaneously, detailed project references from the past five years using a required reference form, lists of available equipment with makes and models, safety records and emergency response plans, and employee resumes with experience and certifications. Vendors must select all applicable treatment categories and acreage sizes they wish to be considered for on the provided attachment; selecting only the highest acreage category is insufficient. No pricing information is included in this technical criteria document, as the RFSQ establishes a pre-qualification list that eligible users will access through separate quotation or bid processes under State Purchasing procedures.

Attachment C Statement of Qualification Technical Criteria

This section of the Vendor's Statement of Qualification should constitute the major portion of the RFSQ. The following specific information must be included in the detailed response:

A. Executive Summary.

The Executive Summary is limited to two (2) pages and is used to briefly describe the Rangeland Firm’s Statement of Qualifications (SQ). This summary should highlight the major features of the SQ. It must indicate any requirements that cannot be met by the Rangeland Firm. The State should be able to determine the essence of the SQ by reading the executive summary. Protected information requests should be identified in this section.

B. Detailed Response.

This section should constitute the major portion of the SQ, not to exceed 25 pages, and must contain: Name of the company and location of company’s principal place of business—including contact person, physical and mailing address(s), phone numbers, website, and email address. The following specific information must be included in the detailed response and will be scored as indicated.

Experience and Demonstrated Ability to Perform (35 Points):

Please provide the following information about your company and its ability to perform this work:

i. age of the company and its average number of employees over the past five years

ii. the number of employees in permanent employment status

iii. the number of employees in a temporary status

iv. the average number of years of experience per employee

v. a copy of any business, contractors or other license required to work in the State of Utah

vi. if applicable, information that will document your ability to handle multiple-separate (at least two) projects at the same time

vii. list of projects in each category, including the acres and vegetation type (i.e. timber, shrub, tamarisk, Russian olive, pinyon, juniper, aspen, etc.), and contact information for projects completed in the last 5 years. Indicate whether you were the contractor or sub-contractor. If you were a sub-contractor, who was the contractor you subbed for? Must use Attachment E (Reference Experience Form), for this information.

viii. Have you ever received a 10-day letter to cure? If yes, explain the circumstances and describe how it was resolved. Include the project name and the project manager.

References (25 Points):

References will be selected from Attachment E (Reference Experience Form). A minimum of 3 references will be checked. References will be checked via email, and a current email address must be provided for each reference. Reference from each treatment category for which you are applying for pre-qualification status may be checked. See Attachment H for an example of the Reference Check Score Sheet.

Equipment (15 Points):

In this section, list the type of equipment available to you— whether the equipment is owned, leased, or rented on a case by case basis. Types of equipment include: chain saws, saw chains, mastication equipment, cutters-head sizes, dozers, chains (Ely or other), chippers, night-work equipment, welding equipment, etc. Equipment makes and models must be included. Summarize your plan for on-site equipment breakdown including repair and/or replacement.

Safety (15 Points):

In this section, include information on your safety record and practices, and summarize your on-site safety and remote area emergency response plan, not to exceed 5 pages. Please include information on any employee who is CPR/First Aid certified/trained or has other medical certifications/experience.

Resumes (10 Points):

Include a list of employees, their years of experience and types of equipment operated. A resume for key personnel (job foreman/superintendent) should be included. In addition to listing experience/qualifications, the resume should also indicate if the employee is bonded and/or able to speak English.

C. Project Categories and Sizes for Consideration:

Use Attachment F (Rangeland Categories and Sizes) of this RFSQ to declare which rangeland categories and size of projects you want to be considered for pre-qualification. General Job Descriptions for each category are included in Attachment C. You MUST check each box in every treatment category and every acreage category you would like to be evaluated for. DO NOT check just the highest acreage - select all acreages for each category you are applying for.

This detailed response shall not exceed 25 pages in length, using a font no smaller than Times New Roman 12-pt. Detailed resumes and other attachments and information included in the SQ will count in the 25-page limit. Copies of any required licenses/certifications, etc. will also be considered part of the 25-page limit. The 2-page Executive Summary, and Attachment G, Rangeland Categories, will not count in this 25-page limit. Do not include company brochures, fliers, pamphlets, magazines, or other printed materials not relative to the specific requirements above. If included, these types of documents will count toward the 25-page limit. Any tab pages, separation pages, or similar dividing pages will also count toward the 25-page limit.

For ease of evaluation, a Vendor’s response must provide a narrative point-by-point response to each evaluation criteria. The criteria are not intended to limit a proposal's content or exclude any relevant or essential data. Vendors are at liberty and are encouraged to expand upon the criteria to demonstrate the Vendor’s capability to provide the State with a solution.

Evaluation Process

Stage 1: Initial Review

In the initial phase of the evaluation process, the conducting procurement unit will review all responses timely received. Non-responsive responses not responding to RFSQ mandatory minimum requirements or unable to meet the mandatory minimum requirements/qualifications will be eliminated from further consideration. The State may request additional information from vendors to clarify responses.

Stage 2: Statement of Qualification Evaluation

Responsive Vendors will then be evaluated against the Statement of Qualifications criteria. Responses will be evaluated against the evaluation criteria as follows:

WEIGHT EVALUATION CRITERIA

35 % Company and Employee Experience 25 % Reference Information 15 % Equipment 15 % Safety 10 % Resumes

A minimum final evaluated score of 70 is required to be pre-qualified on any job category checked on Attachment G. The State will adhere to this threshold. Vendors whose SQ achieves a total score of 70 points or greater for a particular category will be placed on the pre-qualification list for that category. It is possible to be qualified for one or more rangeland categories, but not for others depending on the contractor’s qualifications or limits on the contractor’s qualifications/capabilities. Applicants will only be considered for categories that have been checked on Attachment G.

Stage 3: Rangeland Pre-Qual List

Vendors that met the minimum score threshold in Stage 2 will be added to the Rangeland Pre-Qual list.

In order to assign work under the approved vendor list Eligible Users must use a quotation or bid process through State Purchasing. Pursuant to the Utah Procurement Code, an Eligible User may not assign work to a vendor on an approved vendor list for a procurement item that is outside the scope of the general description of the procurement item contained in RFSQ.
