About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in Los Alamos, New Mexico for the Operation and Staffing of the Performing Arts Center. The form requires offerors to certify their eligibility to participate in the procurement by confirming they are not debarred, suspended, or otherwise ineligible for covered transactions with any federal, state, or local department or agency.

Offerors must certify that neither they nor their principals have been convicted of fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records within the preceding three years; have violated contract provisions or ethical standards; are currently indicted or criminally charged for such offenses; are current or former County employees or immediate family members of County employees or public officials; or have had public transactions or contracts terminated for cause or default within the preceding three years. Any offeror unable to certify these statements must provide written explanation. The form warns that falsification of any statement constitutes grounds for non-consideration of the proposal or rescission of a contract award, though certification of certain statements may not necessarily preclude consideration for award. An authorized signatory must complete the certification with their name, title, date, and signature.
