About this file
This is a Certification Regarding Debarment, Suspension, Ineligibility, Collusion, and Voluntary Exclusion document for a Florida state and local contract opportunity related to a Utilities Computerized Maintenance Management System (CMMS) and Implementation Services. The procurement requires submission of proposals that include both CMMS software and professional services for configuration, implementation, training, and support. Proposals should present a single, coordinated solution with the proposer acting as prime contractor, though the software provider may alternatively serve as the implementation servicer. No specific response dates, due dates, site visits, bidders' meetings, award dates, or contract terms are referenced in this debarment certification document.
The certification requires the contractor to confirm that neither the contractor nor its principals are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation by any Federal department or agency. If the prospective participant cannot certify to these statements, an explanation must be attached to the RFP. Additionally, the certification requires confirmation that no member or agent of the contractor has had access to information that may provide an unfair advantage. The document requires signature and identification of the proposer's authorized official and the date of certification. No pricing terms, set-asides, incumbent information, funding requirements, or financial amounts are specified in this certification document.
