The affidavit applies to the Utilities Computerized Maintenance Management System (CMMS) and Implementation Services procurement. The document requires the contracting entity to attest under penalty of perjury that it does not use coercion for labor or services in any form.

The statute further defines labor as work of economic or financial value and services as acts committed at the behest of, under supervision of, or for the benefit of another, including forced marriage, servitude, and organ removal. The affidavit requires signature, printed name, title, entity name, and date from an authorized representative of the proposing organization.
