About this file
This is a Cost Proposal Attachment (Attachment C) for Solicitation #BW26-1 issued by the State of Utah Division of Purchasing to establish a contract for Breath Alcohol Testing Devices and Presumptive Drug Tests. The RFP seeks vendors to supply evidential breath alcohol testing devices (non-portable), portable breath alcohol testing devices, presumptive drug tests (toxicology), and presumptive drug tests (colorimetric/surface). The solicitation may result in multiple awards based on vendors meeting technical score and total score thresholds. The contract term is five years, and vendors must guarantee pricing for a minimum of 12 months or provide a percentage discount guaranteed for the life of the contract. The cost proposal document contains four separate categories, each with its own evaluation tab, and vendors must complete all yellow-highlighted required fields for any category in which they seek an award. Cost points available total 300 per category, with the vendor offering the lowest total cost in each category receiving maximum points and other vendors receiving points calculated using the formula: Cost Points × (Lowest Proposed Price/Proposed Price).
Each category in the cost proposal requires vendors to include all costs necessary to own and operate the listed products, including equipment, power supplies, training materials, mouthpieces, thermal paper, troubleshooting support, warranty, shipping to zip code 84020, and any other ancillary costs. Vendors must maintain the same percentage discount on MSRP for all brands listed within their proposal. Additionally, vendors must complete a second table (Tables 1b, 2b, 3b) for each category detailing all anticipated fees and charges that may be assessed to eligible users, including shipping fees, maintenance and repair pricing, extended warranty options, and any other services offered. Any fees not listed in these contractual tables cannot be charged to eligible users. The cost proposal makes clear that all pricing information provided becomes contractual upon award, and vendors will be required to provide a full catalog or price list to be included in the final contract. An administrative fee of 0.25% (one-quarter of one percent) applies to this contract.

Instructions
ATTACHMENT C (Cost Proposal) - for Solicitation #BW26-1
PURPOSE - This Attachment is used to evaluate and assign cost points to a vendor's proposal; however, not all requested information in this attachment will be used to assign cost points. All pricing information provided by the vendor will become contractual if the vendor is awarded a contract. Awarded vendors will be required to provide a full catalog or price list, which will be included in the contract. Any % discounts will be for the life of the contract and pricing will be for the duration of the price guarantee period as stated in the RFP.
COST POINTS - Cost points available: 300 (per category). Each category is independent and scored separately. The vendor with the lowest total cost for each category will receive the maximum cost points for that category. All other vendors will receive a portion of the total cost points available based on a percentage of the lowest offered price. The formula to compute the points is:
Cost Points x (Lowest Proposed Price/Proposed Price). Cost points will be added to a Vendor's Technical Score to determine a total RFP score.
