About this file

This is a cost proposal attachment for the Alpine School District's Request for Proposal to establish multiple contracts with qualified vendors providing educational, classroom, office, art, instructional, and related school supplies for K-12 warehouse operations in Utah. The solicitation uses a market basket evaluation method consisting of 15 highly ordered warehouse items with estimated quantities provided for evaluation purposes only. The 15 items include crayons, white glue, permanent markers, dry-erase markers, highlighters, binders, spiral notebooks, report covers, colored pencils, rulers, clipboards, pencils, invisible tape, sticky notes, and tape dispensers. Vendors must provide current catalogs identifying manufacturer/brand, product number or SKU, product description, unit of measure, published catalog price, and effective date. The contract term is five years.

All pricing must be quoted in dollars and cents, rounded to two decimal places, based on the specified unit of measure for each line item, and FOB Destination with Freight Prepaid, including all freight, delivery, handling, fuel surcharges, and transportation-related costs. The District will calculate each vendor's Total Evaluated Market Basket Price using estimated quantities and verified net unit prices, with the responsive and responsible vendor offering the lowest Total Evaluated Market Basket Price receiving maximum cost points; other vendors receive proportionate points using a cost score formula. Vendors must certify that all catalog information, prices, discounts, calculations, units of measure, and product information are complete and accurate, and that each proposed discount represents a minimum discount from the identified, verifiable published catalog or price list that will remain firm and applies to all products within the applicable brand or category except for expressly identified exclusions. No substitutions are permitted except where specifically noted as "Equal or Better Alternate Accepted."

Cost Proposal Instructions

The District will evaluate cost using a market basket consisting of the 15 highly ordered warehouse items identified by the District. Estimated quantities are provided for evaluation purposes only and do not guarantee any purchase volume.

The Offeror must upload a current catalog or provide electronic access to the catalog used to establish the proposed prices and discounts in the Requested Information Section.

The catalog must identify the manufacturer or brand, product number or SKU, product description, unit of measure or package size, published catalog price, and effective date.

The Offeror shall not add, delete, reorder, rename, or hide rows or columns on the Market Basket Price Sheet.

Only the brands and products specifically identified in this solicitation are acceptable. No substitutions.

Prices must be quoted in dollars and cents and rounded to two decimal places.

Prices must be submitted based on the unit of measure indicated for each line item in the bid table.

All pricing must be FOB Destination, Freight Prepaid. The bid price must include freight, delivery, handling, fuel surcharge, and any other transportation-related costs.

Cost Evaluation The District will calculate each Offeror's Total Evaluated Market Basket Price using the estimated quantities and verified net unit prices for the 15 selected items. The responsive and responsible Offeror with the lowest Total Evaluated Market Basket Price will receive the maximum available cost points. Other Offerors will receive proportionate points using the following formula:

Cost Score = (Lowest Evaluated Market Basket Price / Offeror's Evaluated Market Basket Price) x Maximum Cost Points
