About this file

This document provides step-by-step instructions for submitting damage in transit claims for leased vehicles under federal contract 47QMCA23R0076. The instructions outline the process for notifying GSA Fleet of damages within 24 hours, obtaining a signed bill of lading, documenting damage codes and locations with photos, and submitting a completed claim package through a Google form within three business days. The claim package must include vehicle information, delivery details, damage descriptions and codes, preferred repair shop, and photos of damages and the VIN. GSA Fleet will then submit the claim to the vehicle supplier for adjudication and obtain status updates every seven days. Claims are to be resolved within 21 days through repair, replacement, or denial with no right of appeal. The federal contract is for the lease of 2024 Star of Life ambulances from various vehicle suppliers by GSA Federal Acquisition Service on behalf of customer agencies.

i. Please note that you must submit the notated Bill of Lading (BOL)/delivery receipt and photographs of the damages with your initial claims notification

b. Obtain a signed Bill of Lading (BOL)/delivery receipt from the delivery driver with all damages notated prior to the driver’s departure. This is a critical document for claim submission.
