About this file
This is an Insurance Schedule document issued by the Nevada Department of Human Services for the Medical Director procurement (RFP 40DHHS-S3579) at the Nevada Youth Training Center. The schedule establishes comprehensive minimum insurance requirements that the contractor must procure and maintain throughout contract performance and any warranty periods. The contractor must obtain Commercial General Liability coverage with a General Aggregate limit of $2,000,000, Products-Completed Operations Aggregate of $1,000,000, Personal and Advertising Injury of $1,000,000, and Each Occurrence limit of $1,000,000, with the State of Nevada named as an additional insured. Workers' Compensation coverage must meet statutory requirements with Employers' Liability limits of $100,000 per accident, $100,000 per disease per employee, and $500,000 disease policy limit, including a waiver of subrogation against the State. Professional Liability (Errors and Omissions) coverage is required with limits of $1,000,000 per claim and $2,000,000 annual aggregate, with claims-made policies requiring retroactive dates preceding the contract effective date and extended discovery periods of two years post-completion.
All insurance policies must be placed with insurers rated A-VII or better by A.M. Best and licensed to do business in Nevada. The contractor must provide Certificates of Insurance (ACORD form or equivalent) signed by authorized insurer representatives prior to work commencement, with thirty days' notice of cancellation required (ten days for non-payment). The State of Nevada must be named as an additional insured to the full limits of liability purchased, regardless of whether those limits exceed minimums. Contractor insurance must be primary and non-contributory with respect to other available sources. All certificates must reference the State project/contract number and project description and be sent to the designated State Department Representative. Failure to maintain required insurance or provide renewal evidence constitutes material breach of contract. Subcontractors must be included as additional insureds under the contractor's policies or provide separate certificates. Any modifications to insurance requirements require approval from the Nevada Attorney General's Office or Risk Manager through administrative action without formal contract amendment.
