About this file

This is a cost sheet attachment for a request for bid (RFB) issued by Salt Lake City Corporation for the procurement of crushed recycled concrete products. The City seeks qualified contractors to supply two primary product lines: 1" minus screened crushed recycled concrete and 1.5" crushed recycled concrete. Bidders must complete the cost sheet by providing pricing per ton for each product type, along with their company name, physical location, and standard operating hours. The document indicates that award will be made to the lowest responsible and responsive bidder per line item, with the City reserving the right to award to multiple contractors based on operational needs. Additional optional value-added items may be included, such as drop-off services for waste material at contractor locations, though these items will not factor into the formal bid evaluation process.

Pricing evaluation will be determined by combining the bidder's quoted price per ton with a transportation cost calculation based on roundtrip mileage from the bidder's physical location to 865 S Delong Street, Salt Lake City, UT 84104, multiplied by the City's standard operating cost of $4.18 per mile for a 10-wheeler truck. All quoted prices must include materials, labor, supervision, insurance, transportation, delivery, fuel, and any other associated surcharges. Bidders must certify compliance with the City's ethics standards and conflict of interest ordinance (Chapter 2.44, Salt Lake City Code), confirming they have not provided illegal gifts or payments to city officials and will not attempt to improperly influence city employees. The document explicitly prohibits suppliers from offering gifts or promotional items to City employees, with violations subject to suspension or debarment from future City procurement processes.
