About this file

The document is a Cost Proposal Form for Insurance Broker Services issued by Salt Lake City Corporation under Solicitation #SLCi26006. The solicitation seeks a qualified insurance broker to provide comprehensive insurance services for a five-year initial contract term. The proposal form requires offerors to detail their annual broker fees, including potential costs for Workday integration and other services, with costs to be evaluated based on the lowest total proposed costs over the contract lifecycle.

The cost proposal includes specific instructions for pricing, such as requiring all necessary service costs to be included, mandating that zero or blank entries will be interpreted as no-cost services, and prohibiting cost additions outside the proposed schedule. The document outlines commission guidelines, noting that brokers may receive commissions beyond the first year under specific conditions, with an explicit exception for the Public Employees Health Program (PEHP). Offerors are required to provide detailed product descriptions, cost drivers, and product costs, with a warning that deviation from the prescribed format may result in proposal disqualification.

Costing Considerations & Instructions
1. All cells in a yellow highlight allow for a Offeror input.
2. Cost will be evaluated based on the lowest total proposed costs (cell H26 and Highlighted in BLUE).
3. Technical Requirements and Cost Proposals must be reflective of one another. All costs necessary to provide services over the initial contract's five (5) year life cycle must be included on this Cost Schedule form.
4. If zero is entered for any cell or if any cell is left blank, the City will assume that the related services are either provided at no cost or built into another cost item. If a cost category is not applicable, enter N/A.
5. Broker may receive commission beyond the first year the new line of coverage is purchased if: 1. The commission is included in the premium cost that will be paid to the carrier regardless of commission considerations, 2. Can be verified by Broker & Carrier that there would be no inflation of premium for said commission. 3. Distribution of commission is directly from the Carrier that owns the line of coverage to Broker, 4. Upon mutual agreement of Broker, Carrier, and City". The exception to this is Public Employees Health Program (PEHP), Broker will not be eligible to receive commissions on coverages from PEHP.
6. Any cost not provided on this Cost Schedule, will not be allowed into the resulting contract. If you identify a cost that cannot be properly captured in this Cost Schedule, then provide suggestions for modifying this Cost Schedule to the Q&A board for this solicitation.
7. Any deviation from this format may result in disqualification of Offeror's proposal.
