About this file

This is a Suspension and Debarment Certification form issued by LEAD Public Schools for use in connection with a furniture supply, delivery, and installation contract opportunity in Tennessee. The certification requires bidders to confirm their eligibility to participate in federally funded transactions by certifying that neither they nor their principals are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from federal transactions. Bidders must also certify that they have not, within the past three years, been convicted of or had civil judgments rendered against them for fraud, criminal offenses related to public or private agreements, antitrust violations, embezzlement, theft, forgery, bribery, falsification or destruction of records, false statements, or receipt of stolen property. Additionally, contractors must affirm they are not presently indicted or criminally or civilly charged with any such offenses and have not had public transactions terminated for cause or default within the past three years.

The form requires contractors to provide their legal business name, DBA, address, authorized representative information, and contact details. Contractors must sign the certification under penalty of false statements, acknowledging that violations may result in contract termination, suspension, or debarment. If applicable, contractors agree to require all subcontractors, vendors, and lower-tier participants to provide identical certifications before entering into covered transactions. Contractors must immediately notify LEAD Public Schools if they become aware of any information contradicting their certification at any point during the contract period.
