About this file
This is a cost sheet attachment for Request for Proposal RM26-8 Sandbags issued by the State of Utah Division of Purchasing. The RFP seeks to establish a contract for the purchase of sandbags used for flood control for eligible users throughout the State of Utah. The contract term is five (5) years, and the procurement may result in multiple awards based on vendors passing both technical and total score thresholds. Bidders must provide detailed product specifications including sandbag manufacturer, brand, catalog number, denier rating, and UV rating. The cost sheet requires pricing submissions for two distinct categories: empty sandbags for reserve stockpile inventory with delivery within 10 business days, and empty sandbags for immediate on-site shipment with delivery within 48 hours from purchase order receipt.
Pricing must be submitted for tiered quantity brackets ranging from 1-25 bags through 500,000+ bags, with separate unit prices required for each category and quantity tier. All freight, delivery, and other fees must be incorporated into the submitted pricing structure, and no additional costs will be permitted on invoices. Cost evaluation will award up to 200 points per category to the contractor with the lowest total cost, with other contractors receiving proportional points based on the formula: Cost Points × (Lowest Proposed Price/Proposed Price). Vendors must guarantee pricing for one year and any offered discounts for the life of the contract. An administrative fee of 0.0% applies. The cost sheet also requests optional information regarding additional services such as pre-filled sandbag delivery, though such services are excluded from the cost evaluation scoring.

Sheet1 RM26-8 Sandbags
Fill in the yellow highlighted fields. Do not alter this cost schedule. Complete the mandatory yellow highlighted fields, do not leave blank or your cost proposal may be deemed non-responsive. For purposes of a fair cost comparison any cost sheets submitted with alterations may be considered non-responsive and disqualified from further consideration.
Cost points will be awarded based on the total unit price for the quantities entered below for Reserve Stockpile Inventory and for Immediate On-Site.
COST POINTS - Cost points available: 200 (per category). Each category is independent and scored separately. The Contractor with the lowest total cost for each category will receive the maximum cost points for that category. All other Contractors will receive a portion of the total cost points available based on a percentage of the lowest offered price. The formula to compute the points is: Cost Points x (Lowest Proposed Price/Proposed Price). Cost points will be added to a Vendor's Technical Score to determine a total RFP score.
