About this file
This is an attachment to a Request for Statement of Qualifications (RFSQ) issued by the State of Utah Division of Purchasing and the Department of Health and Human Services (DHHS) for workforce development training services. The solicitation (MW26-13) seeks a vendor to provide training specific to the Universal Prevention Curriculum Coordinator Series (UPC-C) professional education program developed by Zili Sloboda, or an equivalent training model approved by the state. The contract term is five years. Submissions must be received by the deadline specified in the mwoodward@utah.gov email record and should be sent to that email address with the subject line "MW26-13 Statement of Qualifications submission." Late submissions will not be accepted. Vendors are instructed not to include pricing in their submissions.
The RFSQ requires vendors to respond to six primary questions: providing resumes of 1-3 key personnel with emphasis on those holding doctorates in behavioral health, research, or education; detailing experience teaching prevention science training and providing participant feedback; demonstrating experience working with governmental entities on substance use prevention policy; describing quality assurance processes for customer service and administrative accuracy; indicating whether the vendor offers UPC-C training or an equivalent evidence-based model with supporting qualifications; and confirming the ability to commit necessary resources to meet the scope of work objectives within the specified timeframe. The vendor with the highest ranking will be selected to negotiate final contract terms at fair and reasonable compensation rates determined through negotiation. If agreement cannot be reached with the highest-ranked vendor, the state may proceed with negotiations with the next-highest-ranked vendor.

Attachment C - Questions for Potential Vendors Training for Substance Use Prevention Professionals
REQUEST FOR STATEMENT OF QUALIFICATIONS
Solicitation # MW26-13
Vendor Name
Questions for potential vendors to answer:
1. Attach resumes of 1-3 key people in your organization who would perform this work, if your company received an “award and intent to contract” under MW26-13 (this procurement). Please emphasize any of your people who hold a Doctorate degree in the fields of behavioral health, research, and/or education.
2. What experience does your company and the key people from question #1 have teaching training on applying prevention science to address substance use and other behavioral problems. Please include which framework(s) or curriculum model(s) were used in these historical training(s) and the composition of the training audience (for example: government employees, teachers, students, the general public, etc.)? Additionally, if you have quotes of post-training participants (giving you feedback on the training), please include 3-4 brief participant quotes.
3. What experience does your company and the key people from question #1 have working for governmental entities and consulting for government professionals who make policy decisions regarding the support and implementation of substance use prevention interventions and policies at the community level? What is your experience with teaching these professionals best practices for evaluating prevention activity efforts and outcomes?
4. How does your company ensure quality customer service and accuracy in administrative tasks (for example: ease of booking training; accuracy in training records; contract compliance; and prompt, accurate invoicing)?
5a. Though this procurement was written with the UPC-C model in mind, the State is open to considering “or equivalent” trainings. Is the training you offer the UPC-C model? Check one of these two options:
| |_| |
| Yes. My company is qualified to train the Universal Prevention Curriculum Coordinator Series (UPC-C) professional education program (developed by Zili Sloboda) to substance use prevention professionals and/or other government workforce development audiences |
| Answer question 5b below |
| |_| |
| No. My company does not train the UPC-C but offers an “or equivalent” training model |
| Answer question 5c below |
5b. If you answered “yes” to question 5a above… What experience does your company and the key people from question #1 have teaching UPC-C to substance use prevention professionals and/or other government workforce development audiences? Additionally, what UPC-C training format(s) are offered by your company (for example: on-line; in-person; instructor-led, etc.) and what makes your trainer(s) qualified to teach the UPC-C model?
5c. If you answered “no” to question 5a above… Which substance use prevention model(s) does your company offer training for? What experience does your company and the key people from question #1 have teaching the proposed model? What training format(s) are offered by your company (for example: on-line; in-person; instructor-led, etc.) and what makes your trainer(s) qualified to teach the proposed model? Additionally, provide a concise description of the proposed “or equivalent” training so that the State can confirm that this proposed “or equivalent” training can accomplish the same desired outcomes as the UPC-C model. Is the proposed “or equivalent” training “evidence-based” and/or referenced on the https://www.samhsa.gov/substance-use/prevention/substance-use-disorders website?
6. Please review the MW26-13 scope of work and think about the estimated time and resources it would take your company to accomplish the SOW as written (or “as modified” for an “or equivalent” training model). Can your company commit the resources it would take to achieve the SOW objectives within the time frame described in the SOW? If your company would like to propose any changes in this regard, please outline the changes here.
