This is an insurance requirements exhibit for a contract between Boulder County and a contractor for Civil Engineering/Land Surveying Services for Phase 2 and 3 of Willoughby Corner. The document specifies that the contractor must provide a Certificate of Insurance prior to commencing work, with Boulder County Housing Authority, Boulder County, and several Willoughby Corner entities named as additional insureds on General Liability, Umbrella/Excess Liability, and Pollution Liability policies. Insurance policies must provide 30 days' prior written notice for cancellation (10 days for non-payment of premium), and the contractor must notify the County within 7 days if insurers refuse to provide required notice.

The contractor must maintain several types of insurance coverage at their own expense, including Commercial General Liability with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate, Automobile Liability at $1,000,000 per accident, Workers' Compensation at statutory limits with Employer's Liability minimums of $100,000 per accident, and Professional Liability (Errors and Omissions) at $1,000,000 per loss and aggregate. All deductibles are the contractor's responsibility, the contractor's coverage must be primary over any County insurance, and all policies must include subrogation waivers in favor of the County and its entities. Professional liability coverage may be written on a claims-made basis but requires either continuous coverage or a two-year extended discovery period after contract completion.
