About this file

This is an Affidavit Regarding Labor and Services document issued by the Florida Department of Transportation (FDOT) for the Fort Myers Downtown Pedestrian Mall and Micromobility Feasibility Study (Project 375-030-31). The affidavit is required effective July 1, 2024, pursuant to Florida Statutes Section 787.06(13), and must be executed when a contract is entered into, renewed, or extended between a nongovernmental entity and a governmental entity. The document requires completion of standard fields including the nongovernmental entity's name, address, phone number, authorized representative's name, title, and email address. The affidavit must be signed by an authorized officer or representative of the nongovernmental entity under penalty of perjury, attesting that the entity does not use coercion for labor or services as defined in Florida law. The document must be notarized, with the notary certifying the date of execution and indicating whether the signatory was personally known or produced identification.

The affidavit template does not contain specific pricing terms, set-asides, funding amounts, or other contractual details, as it serves solely as a compliance certification document. This form is a mandatory requirement for all nongovernmental entities entering into contracts with Florida governmental entities and is intended to ensure compliance with human trafficking prevention statutes. Contractors must retain completed, notarized affidavits as part of their contract execution documentation and may be required to resubmit the affidavit if contracts are renewed or extended.
