About this file

The document is a Consultant Schedule Evaluation Guidelines Worksheet from the Florida Department of Transportation (FDOT), designed to assess a consultant's performance on project schedules and submittals. The two-page evaluation form provides a structured method for FDOT project managers to rate consultants across two primary criteria: Project Submittals (weighted at 70%) and Status Reports/Schedule Updates (weighted at 30%). The evaluation uses a 5-point performance rating scale ranging from 1 (Unacceptable Performance) to 5 (Outstanding Performance), with specific performance measure guidelines for each rating level.

The evaluation form requires detailed scoring and comments for each performance criterion, focusing on the consultant's ability to maintain critical path items, meet submittal dates, provide timely schedule updates, and offer suggestions for managing project delays. The document is intended to be used as backup documentation for drafting and discussing performance evaluations, with scores to be transferred to FDOT's internal Consultant Evaluation (CE) application. The form emphasizes the importance of thorough documentation, requiring comments for ratings below 3 and above 4, and serving as a comprehensive tool for tracking and assessing consultant performance on transportation-related projects.
