This document is a past performance questionnaire for evaluation of a contractor's performance on a previous Department of Veterans Affairs project. The questionnaire requests information on the contract such as title, number, type, location, award and completion dates, original and final prices. It asks the evaluator to rate the contractor's performance on quality, schedule, communication, management, cost, safety, and subcontract management. The evaluator is also asked if they would hire or work with the contractor again and provide an overall rating. Space is provided for comments. Upon completion, the evaluator is to sign and date the questionnaire and return it by the proposal due date.

This notice announces a tiered evaluation pre-solicitation for replacement of underground utilities at the White River Junction VA facility in Vermont. The project scope includes general construction, alterations, roads, walks, grading, drainage, water service, sanitary and storm sewers. The solicitation will be a request for proposal evaluated according to FAR Parts 15, 19, and 36. The acquisition is set aside in priority order for SDVOSB, VOSB, HUBZone/8a, and other small businesses. The performance period is approximately 545 days from notice to proceed. The construction value is estimated between $5-10 million. Questions must be submitted in writing by the issue date in December 2019.
