REQUIREMENT PER THE STATEMENT OF WORK

(a)
Contractor creates grave plotting maps that show specific locations of each marker by actual number and name. Map is verified with COR.
[ ]
[ ]
(b)
Section keystones have been properly chosen with the approval of the COR in order to achieve a smooth, flowing transition between the rows and columns.
[ ]
[ ]
(c)
Markers are removed from their sockets and placed on their gravesites. (Markers are not to be removed from their respective gravesites.)
[ ]
[ ]
(d)
Remove the existing plastic marker support system, leveling sand, and aggregate base.
[ ]
[ ]
(e)
Subgrade is ripped to a depth of 6-inches; soil buster (gypsum equivalent) and compost are applied and tilled to a depth of 6-inches. Subgrade is leveled and compacted leaving depth for 2 to 3-inches of imported topsoil to establish finish grade.
(f)
A continuous trench is dug underneath the row of markers to accept the marker supports system, and the subgrade is compacted to 95% maximum dry density.
[ ]
[ ]
(g)
3/4” minus crushed aggregate base is installed to design grade under the plastic support geogrid at marker locations and compacted to 95% maximum dry density.
[ ]
[ ]
(h)
Amended topsoil is installed to design grade underneath the plastic support geogrid where sod is to be planted and compacted to 90% maximum dry density.
[ ]
[ ]
(i)
Plastic marker support geogrid is installed in continuous length for each row.
[ ]
[ ]
(j)
1/4” minus crushed aggregate leveling sand is installed and compacted in the cells of the plastic marker support geogrid at marker locations.
[ ]
[ ]
(k)
Markers are correctly installed on plastic marker support geogrid: set in a line laterally, transversely, and diagonally with flat markers of other graves, and the top surfaces at proper heights and levels to provide a uniform flowing transition through the rolling terrain.
[ ]
[ ]
(l)
Markers are verified to be reinstalled on the correct gravesites using temporary grave plotting maps, existing permanent control markers where available, and temporary contractor installed control markers precisely installed at the ends of each gravesite row
[ ]
[ ]
(m)
Two to 3-inches of Imported topsoil is installed and compacted in the cells of the plastic marker support geogrid. Two to 3-inches of imported topsoil is placed throughout the entire renovation area where sod is to be planted.
[ ]
[ ]
(n)
Markers in all completed work areas are firmly set and anchored in place with no movement from forces subjected by the COR and correct in appearance, spacing, depth, alignment, plumbness, height, accuracy, and smoothness in grade transition.
[ ]
[ ]
(o)
Markers are cleaned using headstone cleaning agent and pressure washing.
[ ]
[ ]
(p)
Any contractor caused damage is repaired according to this Work Statement.
[ ]
[ ]

The entire vegetated area has been treated with an application of a non-selective and selective herbicides by a licensed pesticide applicator. A minimum of 10 to 14 days has elapsed for the herbicide to fully translocate throughout all plant parts. The COR confirms that all vegetation in the treated area is completely dead.

The entire treated area has been power raked or verti-cut and all residual plant debris including thatch has been removed.
[ ]

The entire turf renovation area has been treated a second time with an application of a non-selective and selective herbicides by a licensed pesticide applicator. A minimum of 10 to 14 days has elapsed for the herbicide to fully translocate throughout all plant parts. The COR confirms that all vegetation in the treated area is completely dead.

Imported topsoil applied as needed. Fertilizer and soil amendments are broadcast onto the topsoil per soils report and thoroughly rototilled into the soil to a minimum depth of 6 inches.
[ ]
[ ]
(g)
Amended topsoil has been graded, leveled, compacted to 85% maximum dry density, and raked to achieve a finished appearance that is smooth, uniformly level, and free of all surface ripples, depressions, high spots, low areas, ridges and stones larger than 1/2-inch. The finish grade is a maximum of the sod thickness below the adjoining grade of any finished surface and is nominally 1-inch below the top of all flat markers unless otherwise approved by the COR.
[ ]
[ ]
(h)
Irrigation equipment is re-installed, and irrigation system tested, repaired, and determined fully functional.
[ ]
[ ]
(i)
Irrigation heads and valve boxes are brought to proper relationship with finish grade with tops of heads and valves boxes at 1-inch above finish grade.
[ ]
[ ]
(j)
Turfgrass starter fertilizer has been uniformly broadcast onto the prepared sod bed with a properly calibrated fertilizer spreader at the rate specified for the specified Turf-Type Tall Fescue.
[ ]
[ ]
(k)
All turfgrass sod has been installed using certified sod as specified. Sod pieces are not stretched or overlapped and that all joints are butted tightly to prevent voids that would cause air drying of the roots and to prevent weed growth, and lightly rolled to ensure proper soil contact.
[ ]
[ ]
(l)
Turf is properly maintained during Turf Establishment Period as described in the Work Statement: weeds, diseases, pests eradicated, bare areas re-sodded, area correctly mowed, trimmed, irrigated.
