ATTACHMENT 21
From: Houston, Jackie M Sent: Thursday, April 17, 2008 5:22 PM To: VHA OI IA MPI DQ Team Subject: Test Patient Records Guidelines Issue: Records that appear to have been entered as a real patient and then changed to a test patient record. Record now has suspect clinical data that may or may not be test data and unable to make that determination. Record may also have an ESR correlation attached.
Test patient records with no clinical data - The test patient record needs to be restored with the real patient’s id traits and duplicate records can be merged.
Test patient records with clinical data – If can determine that there is real patient clinical data then record needs to be restored and merged. If there is more than 2 pieces of clinical data that you are unable to determine if for the real patient then record needs to remain as test patient record and all real patient data needs to be removed from the record. If clinical data for the real patient can be identified then it needs to be moved/added? into the duplicate record for the patient as I would think there is normally another record for the patient at the site.
ESR Correlations – To remove an ESR Correlation from a test patient record send to HEC to remove that ICN from their record. After you verify that the ICN has been removed then send Danny & Chris Link a message to remove the ESR Correlation from that ICN. I think we need to have one on-going message for this.
Everyone needs to run these by Carol or Shelley for a while until we can work through this process to ensure this is the correct way to handle these.
Carol and Shelley – please add any other information that I have forgotten. Thanks!
