B.2 PRICE/COST SCHEDULE
NOTE 1: Offeror’s prices shall consist of a handling fee per line item only. The handling fee shall include all the offeror’s costs such as labor, overhead, shipping cartons, any required labels, packing materials, etc. Payment to the Contractor for each line item dispensed will be at the current Federal Government Federal Supply Schedule (FSS) contract price in effect for the item at the time the order is released to the Contractor. Payment for shipping will be accomplished through use of an account number that will be provided by the VA for that purpose, and shipping costs will not be borne by the Contractor. Miscalculations by the Contractor in manifesting parcels are the responsibility of the Contractor and will not be passed to the Government for payment. The Government will be auditing all shipping manifests and invoices to ensure the Contractor follows all requirements.
NOTE 2: Upon award, the Contactor shall work with CMOP to mitigate shipping costs. The Contractor shall be able to manifest parcels internally through different vendors (USPS and third-party vendors) and/or (at the discretion of the CMOP) work with CMOP’s mail consolidators to distribute parcels to the Veteran. The Contractor shall release parcels daily to ensure parcel distribution and critical entry times are managed appropriately to meet service parameters. The Government is considering the use of a CMOP mail manifesting contract for shipment of packages under this contract. This action may be taken at time of contract award or by modification to the contract following contract award.
NOTE 3: Vendors are not required to submit pricing for all line items to be considered. Offerors may submit a proposal for all Med/Surg Items only (to include all option periods), all of Enteral nutrition and consumable med/surg items (to include all option periods), or all line items. To be eligible for award consideration, offerors are required to submit a proposal for the base period of one year and each of the four one-year option periods for each line item proposal. Proposals that do not offer a handling fee for the base period and each of the four option periods for the solicitation line item being offered, shall be determined non-responsive.
NOTE 4: Vendors should fill out Attachment 8 (Pricing Spreadsheet) and submit with offer.

B.3 PERFORMANCE WORK STATEMENT
A. BACKGROUND
The Department of Veterans Affairs (VA) Consolidated Mail Outpatient Pharmacy (CMOP) program serves over seven million enrolled Veterans, dispensing more than 120 million prescriptions per year for Department of Veterans Affairs Medical Centers (VAMCs) and Community Based Outpatient Clinics (CBOCs) throughout the United States. Currently there are a total of eight nationally located CMOP facilities. CMOP has identified the continued need to provide medical/surgical (med/surg) and enteral nutrition items from a contracted source via receipt of electronic orders for direct delivery to Veterans. Prescriptions (Rxs) may contain multiples of a single item.
Contractor distributed med/surg items play a vital role in the fulfillment process by improving the efficiency of CMOP operations creating faster delivery times to our Veteran patients. Fulfillment fluctuations for these items make it difficult to determine the exact quantities that will be required during the contract term(s); however, based off historic data the Government anticipates a need for approximately four million-line items per year.
B. SCOPE
The Contractor is to provide med/surg supplies and enteral nutrition items for direct delivery to patients located within the fifty United States, Washington DC, Puerto Rico, American Samoa, Northern Mariana Islands, US Virgin Islands and Guam in accordance with addresses supplied. The Contractor is expected to maintain sufficient inventory at all times to meet CMOP requirements.
1. The Contractor will be reimbursed based upon the number of line items filled for each order. All orders shall be filled based upon information provided within each order issued from CMOP per the Performance Work Statement. Section D Attachment 1 Sample List of Products Distributed as of the last Quarter, represents a sample of the type and volume of products that will be dispensed. This list is only representative of one snapshot in time and is subject to change at VA discretion.
2. All actions by the Contractor under the awarded contract shall be in accordance with applicable Food and Drug Administration (FDA), Drug Enforcement Administration (DEA), Environmental Protection Agency (EPA), Federal, State and Local codes and laws or regulations, including renewal of required (Federal or State) certifications, as applicable. The Contractor shall meet any current or future requirements for approvals or certifications as may be levied by applicable legal, regulatory, accrediting or local/state/federal requirements for the fulfillment of medical/surgical and enteral nutrition items, or, any additional agencies as identified by the Government to meet specific requirements. Due to continuous regulatory changes in both federal and state laws, the Contractor shall comply with new regulations within the date stated in the law, or regulation.
3. The Contractor shall have a minimum of two years’ experience in the provision and shipment of home health care supplies with a minimum annual volume of at least four million-line items per year. The Contractor must have multiple warehouse distribution operations to maintain full functionality in the event of a disaster, either natural or manmade, which includes technological disasters, such as power service disruptions and cyber-attacks.
CONTRACT PERIOD
1. The contract performance period will be a base of one (1) year, and four (4) one-year option periods.
i. Base Year: 01-01-2021 to 12-31-2021
ii. Option Period 1: 01-01-2022 to 12-31-2022
iii. Option Period 2: 01-01-2023 to 12-31-2023
iv. Option Period 3: 01-01-2024 to 12-31-2024
v. Option Period 4: 01-01-2025 to 12-31-2025
2. The Implementation/Transition Period is defined as the amount of time after contract award and before the contract effective date to ensure sufficient administrative and training time has been allowed for continuity of service in support of the CMOP program, and, if necessary, the change of Contractors from the incumbent Contractor to the newly awarded Contractor. The Implementation/Transition period shall be no more than one hundred-twenty (120) calendar days from date of contract award for a newly awarded Contractor, or, ninety (90) calendar days from date of contract award if award is made to the incumbent Contractor.
PREPROPOSAL CONFERENCE
1. A preproposal conference will be held by the Department of Veterans Affairs to explain the requirements of this solicitation, and to respond to general questions raised by prospective offerors. Interested offerors wishing to attend the conference must notify, the Contract Specialist, Jeffrey Crysler (jeffrey.crysler@va.gov) of the number and names of attendees no later than (09/22/2020). The preproposal conference will be held virtually through Microsoft Teams (tele-conference) on (09/25/2020). The Contract Specialist will send out a meeting invite once names of attendee’s are received. Written minutes of tele-conference will be provided to all prospective Offerors via formal written amendment to the Request for Proposal (RFP) on Contracting Opportunities, formerly Fed Biz Opps.
2. Offerors are requested to submit written questions concerning this solicitation to arrive on or before (09/22/2020). Interested offerors unable to attend the conference are also invited to submit their written questions by the requested date. All questions that are submitted together with their respective answers will be furnished to all prospective offerors attending the conference or to interested parties upon written request. Written questions should be submitted to jeffrey.crysler@va.gov. Offerors are advised that any remarks or explanations provided at the preproposal conference shall not change the terms and conditions of the solicitation unless the solicitation is amended in writing.
C. IMPLEMENTATION/TRANSITION PERIOD
1. CMOP will provide the Contractor, at contract award conference, a line item usage report for the most recent contract quarter. The report will include a list of specific products, identified by the example contract item number and example contract number, for the items distributed to patients, along with the current FSS, National Standardization, or SourceAmerica price for each. The usage data report is fluid and may be periodically updated as product distribution requirements change.
2. The Contractor shall provide a written Implementation/Transition plan to minimize any decreases in productivity and to prevent possible negative impact on service. This plan shall include but is not limited to the identification and establishment of new processing facilities and locations, the hiring, and training of staff prior to the start date, and establishment of points of contacts. The Implementation/Transition Plan shall be included in the proposal and shall be incorporated in the contract award.
3. The Contractor shall implement Health Level 7 (HL7) electronic data interface with CMOP (see Section D, Attachment 3, CMOP Vendor Interface Specification). This must be initiated punctually upon the inception of the implementation Period, since it requires customized software creation that may be required by the Contractor.
