About this file
This document contains an amendment to a solicitation for Community Based Outpatient Clinics to be operated by a contractor on behalf of the VA Loma Linda Healthcare System. The amendment provides clarification on several questions received from offerors in response to the original solicitation. Key details include the monthly volume of Choice/Mission Act referrals over the past year for each of the five clinic locations broken down by primary care, behavioral health medication management, mental health, and psychotherapy. The amendment also specifies that podiatry exam rooms and on-site podiatry services are not required, as patients will be referred to the VA medical center for specialty care. Offerors are directed to include staffing plans for each clinic in the responsibility determination section of their proposals rather than the experience factor. Proposal submission requirements and deadlines are also clarified.

2. Please provide data for medication and vaccine usage for these Choice/Mission Act referral patients.
The VA does not have this information.
3. RFP, Section 22.20: Does this mean the contractor does not need to supply a podiatry exam room or nail technician, and all patients will be referred and travel to Loma Linda VAMC for podiatric care?
A Podiatry exam room is not required, patients will be referred to the VA for all specialty care services.
4. Per Amendment 0002, Question 73, can the VA provide usage data on the volume of over-the-counter medications distributed at each CBOC in the past year?
The VA does not have this information.
5. Per Factor 2, what specific detail needs to be provided on the IT infrastructure. It is difficult to provide this information because our access to the facilities, in some cases, is limited.
For Factor 2, the offerors are to describe, for each of the five facilities, the offeror’s proposed physical resources including number/type of patient care areas, offices, information technology infrastructure, parking facilities, parking for employees and handicapped parking that the offeror will put in place to meet the requirements of the solicitation. Concerning the proposed information technology infrastructure, offerors should provide the level of detail, that the offeror believes is sufficient to describe how the offeror plans to meet the information technology needs described in the solicitation.
Proposal document Structure
6. Per Amendment 0002, Questions 99, 116, and 117 please further clarify where the General Responsibility Determination responses should be provided in the document. The answer references the RFP-page 134, section b, (2) and states that these responses are not part of the evaluation factors. Yet page 134 lists them as part of the evaluation factors.
The Information requested for the General Responsibility Determination is not part of the evaluation factors. This information shall be submitted in accordance with the format provided below. This information was included in Amendment 0003 – Page10.
1. Addendum to FAR 52.212-1 Instructions to Offerors -Commercial Items. 1. b. (2) is deleted in its entirety and is replaced with the following:
(2) Past Performance, Veteran Involvement and Price Proposal and Responsibility- Offerors shall submit a separate binder with the information the solicitation requires offerors to provide regarding the following items:
0. Past Performance evaluation
0. Veteran Involvement evaluation
0. Price evaluation
0. Responsibility Determination.
0. Signed 1449, SF30 Amendment(s), and Contractor Data Sheet Only one paper copy is required, but all shall be placed in one binder clearly marked with the Offeror’s name, Point of Contact, Phone Number and email address, and the RFP number. Additionally, one CD with an electronic version of all documents contained in this binder shall also be included in the binder.
7. Since the General Responsibility Determination includes the staffing plan, does this mean that the organizational chart/staffing plan for each clinic does not need to be included in the experience/management (Factor 4) section of the proposal?
The staffing plan for each of the clinics shall be included in the information requested under the General Responsibility Determination.
8. Does the VA want responses for Factors 2 and 3 in a separate binder for each of the five clinics? This would mean that a total of 30 binders (six copies each) would have to be submitted for these sections alone.
The responses can be in the same folder separated by tabs for each of the five (5) clinics.
9. Where should the signed amendment acknowledgements be placed in the proposal document? Typically, RFPs instruct them to be included in the Pricing volume, but the RFP does not state this.
This information was included in Amendment 0003 – Page10.

10. When working with our architects, we found that the government deleted the requirement for a separate ventilation system for mailrooms in the RFP. However, the government did not delete the requirement for mailrooms from section 1.02.4.2 of the RFP (requirements for Murrieta Audiology). Please advise if the government’s intention is to have any mailrooms in the CBOCs. Please extend proposal deadline to give offerors time to update their architectural floor plans.
The requirement in the Murrieta CBOC is for a 120-sf room for VA to place an all in-one copier with a counter for mail slots and an area to sort documents and distribution, which is standard in most VA administrative operations. This is for the VA employees and for mail drop off and pick up via VA courier. No separate ventilation system is required.
11. Section: Space Requirements, 1.02.3 (13) Does the Vestibulography room require any specialized construction to accommodate this testing?
The room does not require any specialized construction for testing other than what’s stipulation in the RFP.
