About this file
This document is a Release Identification and Work Plan for Additional Environmental Site Assessment (ESA) prepared by UES for the Ioannis A. Lougaris VA Medical Center in Reno, Nevada. The report details the discovery of an abandoned underground storage tank (UST) during construction activities in December 2023, with soil samples revealing total petroleum hydrocarbon (TPH) contamination exceeding Nevada Division of Environmental Protection (NDEP) screening levels. The diesel tank, believed to have been used for a former facility boiler and abandoned in the late 1970s, was found near the laundry loading dock of Building 1, with soil samples showing TPH diesel range organics (DRO) concentrations of 1,147 mg/kg and 1,578 mg/kg, and oil range organics (ORO) concentrations of 620 mg/kg and 1,000 mg/kg.
The work plan outlines a comprehensive approach for further investigation and remediation, including UST removal during planned 2025 construction activities. Key steps include opening and inspecting the UST, removing residual product, conducting mechanical excavation, collecting confirmation soil samples from the bottom and sidewalls of the expanded excavation, and analytical testing for TPH, volatile organic compounds (VOCs), and polynuclear aromatics (PNAs). UES will notify NDEP prior to UST removal and prepare a detailed report of findings, with the ultimate goal of characterizing and addressing the petroleum contamination at the site. The project is being conducted under NDEP Spill Report #240205-01, filed on February 5, 2024.

6.0 CONCLUSIONS
TPH results detected in the soil samples collected by RK personnel indicate that petroleum impacts exist in the soils proximal to the orphaned UST. The soil sample analycal results indicated TPH DRO and ORO concentraons up to 1,578 and 1,000 mg/kg, respecvely, at approximately 7.25 feet bgs which exceeds the NDEP screening level of 100 mg/kg. Based on the discovery of impacted soils and the analycal results, UES opines that addional assessment and remediaon acvies are warranted.
7.0 WORK PLAN FOR ADDITIONAL ESA
Pursuant to the discussion with the Department of Veteran affairs and the NDEP, the UST is proposed to be removed during future construcon acvies that are tentavely planned for 2025. The proposed construcon project includes a tunnel that connects the parking garage located across Kirman Avenue, to the exisng building proximal to the UST. The locaon of the tunnel alignment would require the UST to be removed.
7.1 SCOPE OF WORK
The addional ESA acvies will include the following:
• Overseeing excavaon and removal of the UST;
• collecng confirmaon soil samples from beneath the UST;
• field assessing the extent of soils impacted by the release;
• overseeing excavaon of impacted soils as necessary;
• collecng confirmaon soil samples following excavaon acvies as applicable;
• analycal tesng of the collected samples by a Nevada-cerfied laboratory;
• disposing of impacted soil; and,
• preparing a report of findings.
7.2 PRE-FIELD ACTIVITIES
Prior to removal, the UST will be opened and inspected to determine if the UST was filled with an inert substance or if product is remaining. Residual product will be pumped, if necessary, and the tank will be triple-rinsed and inerted of flammable vapors to lower the explosive limit and/or oxygen to a safe level for removal. Rinsate/residual product will be collected for offsite disposal.
7.3 EXCAVATION AND REMOVAL OF THE UST
The UST will be excavated and removed using mechanical equipment. Tank removal will be conducted by a Nevada-licensed tank handler. Upon removal, the UST will be visually inspected to determine condion and signs of corrosion holes or damage.
7.4 INITIAL SOIL SAMPLE COLLECTION
Following removal of the UST, one soil sample will be collected from each end of the UST within two feet of the botom of the tank in nave soils. Samples will be collected directly from the excavator bucket for safety. Soil samples will be collected in laboratory provided containers, sealed, labeled, and preserved on ice pending shipment to the laboratory.
7.5 CONTAMINATED SOIL EXCAVATION AS NECESSARY
Following removal of the UST and inial sample collecon, the UST excavaon will be expanded laterally and vercally to remove grossly impacted soil. During excavaon acvies, soils will be screened for petroleum contaminaon using visual and olfactory senses. Excavaon will be conducted using mechanical excavaon equipment unl field screening indicates that the grossly impacted soils are removed and/or addional excavaon is not feasible or safe due to onsite constraints such as ulies and exisng structures. Excavated soil will be placed directly in roll off bins or stockpiled on visqueen pending transport to the waste disposal facility.
7.6 CONFIRMATION SOIL SAMPLE COLLECTION
Confirmaon soil samples will be collected from the botom and sidewalls of the expanded excavaon at the rate of one per floor and one per sidewall. In the event the floor or sidewalls exceed 100 square feet in area (each), an addional sample will be collected every 100 square feet or poron thereaer. Sidewall samples will be biased towards areas of noted impacts or the elevaon of the botom of the UST. The soil samples will be placed in the laboratory provided glass sample containers with Teflon lids, sealed, labeled, and preserved on ice in a cooler pending delivery to the laboratory.
7.7 ANALYTICAL TESTING
Collected soil samples will be delivered under chain-of-custody protocol to Pace Analycal (Pace) of Mount Juliet, Tennessee for analycal tesng. The soil samples will be analyzed for Full Suite TPH by EPA Method SW8015 and selected samples will be analyzed for VOCs by EPA Method SW8260 and polynuclear aromacs (PNAs) by EPA Method SW8270, based on TPH results.
7.8 SCHEDULE AND REPORT PREPARATION
Construcon and subsequent UST removal acvies are currently slated for 2025. UES will nofy NDEP at least one week prior to UST removal via email. Following the compleon of field acvies, UES will prepare a report of findings for submission to the NDEP. The report will include a descripon of field acvies, a site map showing sample locaons, supporng laboratory documentaon, and recommendaons.

Media Affected: Soil If UST, Confirmed Visually? No
Cause of Complaint/Spill:
A boiler heating tank was abandoned in place in the late 1970's, unknown to current facility engineering staff until recent digging for an unrelated construction project took place in December 2023. Contamination was suspected due to a VOC odor after the initial digging, but was unknown until soil samples were taken on 1/22/2024 by a subcontractor for the construction project, CEM Ryan Kautz of Rk Contractors, to confirm contamination.
Remedial Action Taken:
Reno VA Hospital has contacted CEM Caitlin Jelle from UES (formerly McGinley & Associates) on 2/5/2024, and scheduled a site walk for tomorrow to discuss plans for a site assessment.
