1.0 General Information and Inspection Findings
This Asbestos Management Plan (AMP) was prepared following procedures identified in the EPA Asbestos Hazard Emergency Response Act (AHERA) (40 CFR Part 763 Subpart E). Though AHERA applies to schools (K-12) and the Ioannis A. Lougaris Medical Center (Medical Center) is a health care facility and not a school, the Sierra Nevada Healthcare System of the Veterans Administration (VA) has chosen AHERA methods for its asbestos management procedures. In addition to the requirements of AHERA, this AMP will address previously identified Asbestos Containing Materials (ACM) in addition to the AHERA required Asbestos Containing Building Materials (ACBM) which are a subset of ACMs. ACM is defined by the EPA and OSHA as those materials containing greater than one (1) percent asbestos.
During the week of April 13 through 17, 2009, with a follow-up site visit on April 7th , 2010, Converse Consultants (Converse) conducted a visual re inspection of buildings 1, lA, 1 D, 5, 7, 8, and 10 located at the Medical Center, 1000 Locust Street, Reno, Nevada for the current presence and condition of asbestos-containing building materials (ACBMs). This Asbestos Management Plan (AMP) includes information from the previous Wise Consulting and Training AMP dated June of 1999 and the data identified in the Asbestos Re Inspection and Assessment (ARA) report by IHI Environmental (IHI) dated December 7, 2001. The IHI ARA was based on an Asbestos Survey Report dated August 1992 by National Asbestos Consultants, Inc.
The following tables summarize the currently assessed building materials from the Medical Center buildings that have been evaluated. The first set of tables describes the ACMs that have been identified. The next category of table describes non-ACM materials identified that had a trace of asbestos detected.
Though the EPA would not regulate these materials, OSHA would continue to require employees not to be exposed to asbestos over the airborne exposure limit if these are disturbed. As such these materials should be included in the APM. The last table summarizes those materials that have been sampled and analyzed and found not to have detectable asbestos present.

1.3 Sampled Building Materials With No Detectable Asbestos
The following Tables summarize the homogeneous areas and locations of building materials that were sampled and found not to contain detectable asbestos. It should be noted that current industry practices have identified non-friable organically bound (NOB) materials such as floor tile and mastics as being difficult to analyze using the traditional Polarized Light Microscopy (PLM) method. As such NOBs that have been found not to contain asbestos by PLM should be considered ACM until at least one representative sample is analyzed by Transmission Electron Microscopy (TEM) to confirm the absence of asbestos.

AHERA Triennial Asbestos Re-inspection loannis A. Lougaris Medical Center Clinic - Building 1 D 1000 Locust Street Reno, Nevada
Dear Mr. Luscavage:
09-23635-01
In compliance with the Asbestos Hazard Emergency Response Act (AHERA) guidelines, Converse Consultants (Converse) has conducted an AHERA Asbestos Re inspection at the Clinic designated as Building 1 D at the loannis A. Lougaris Medical Center in Reno, Nevada. The re-inspection was performed during the period of April 13th through April 17th, 2009 with a follow-up site visit on April ?1h, 2010. Although the loannis A. Lougaris Medical Center is a health care facility, and not a school, the Sierra Nevada Health Care System of the Veterans Administration (VA) has chosen AHERA methods as policy for asbestos management procedures.
During the course of our investigation, Converse reviewed the original AHERA inspection report issued by National Asbestos Consultants in August 1992, the Asbestos Re-inspection Report dated December 2001 conducted by IHI Environmental and the Asbestos Management Plan established by Wise Consulting & Training dated June 1999. Converse also reviewed available surveillance information for review to determine compliance and whether any response actions including abatement and/or Operations and Maintenance (O&M) had been conducted.
No changes in material descriptions and/or assessment categories have been made in regards to any ACBMs and/or presumed ACBMs located at the site.
The records indicated that abatement had been conducted in regards to asbestos containing building materials (ACBMs) and/or presumed ACBMs. The areas of loannis A. Lougaris Medical Center
May 14, 2010 abatement including the materials abated and the quantity of those materials was identified as the following:
• The removal of approximately 1,000 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the areas designated as GD111, GD112, GD113 and GD115. (July 2008)
• The removal of approximately 50 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the area designated as GD120. (July 2008)
• The removal of approximately 60 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the area designated as GD100. (July 2008)
• The removal of approximately 5,600 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the areas designated as GD211, GD213 and GDC15. (December 2008)
• The removal of approximately 750 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the 1st Floor in the area designated as 1D162.
(November 2007)
• The removal of approximately 220 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the 1st Floor in the area designated as 1D144.
(date of removal unknown)
In regards to homogenous materials located at the site, seven (7) additional suspect ACBMS were identified in addition to those which were previously identified. These materials consisted of the following:
• Vinyl Sheet Floor Covering
• Base Cove/Mastic
• Drywall/Joint Compound
• Ceramic Tile/Grout/Mortar (walls)
• Ceramic Tile/Grout/Mortar (floors)
• Exterior Wall Surfacing (Stucco)
• Roofing Material/Mastics

May 14, 2010
These materials have been added to the O & M Program including the 6-month periodic surveillance as presumed ACBMs until testing has been conducted to determine whether the materials contain asbestos.
Given the relatively good condition of the ACBMs and/or presumed ACBMs within the structure, Converse recommends the continued implementation of the existing O&M Program. It must be noted that all documentation concerning future remediation of ACBMs must be updated in an on-going process and copies of all documented abatement must be kept with the O & M manual. Further information is identified in the attached "AHERA Asbestos Re-inspection" report.
Thank you for the opportunity to be of service. Should you have any questions regarding this report, please do not hesitate to contact our office.
Respectfully Submitted, CONVERSE CONSULT ANTS

INTRODUCTION
AH ERA ASBESTOS RE-INSPECTION
loannis A. Lougaris Medical Center Buildings 1 D
In compliance with the Asbestos Hazard Emergency Response Act (AHERA) guidelines, Converse Consultants (Converse) has conducted an AHERA Asbestos Re inspection at the Clinic designated as Building 1 D at the loannis A. Lougaris Medical Center (VA-Reno) in Reno, Nevada. The re-inspection was performed during the period of April 13th through April 17th, 2009 with a follow-up site visit on April yth, 2010.
Although the loannis A. Lougaris Medical Cen'ter (VA-Reno) is a health care facility, and not a school, the Sierra Nevada Health Care System of the Veterans Administration (VA) has chosen AHERA methods as policy for asbestos management procedures.
SCOPE OF SERVICES
The scope of services consisted of a visual re-inspection of all known asbestos containing building materials (ACBMs) and/or presumed ACBMs present at the facility for the purpose of assessing the condition of those materials and to determine whether any response actions are required in regards to those ACBMs. The scope also included a review of the existing Management Plan, documentation of any abatement regarding ACBMs and a review of the data collected in regards to the 6-Month Periodic Surveillance Program.
METHODS
Converse provided the professional services of Steve Havens, Andrew Kirk, Lynn Minedew - Inspectors and John W. Petersen - Management Planner, to review the asbestos re-inspection report dated December 2001 for the buildings conducted by IHI Environmental and the original AHERA inspection conducted by National Asbestos Consultants dated August 1992. Also, a review and update was conducted by Dale Walsh, CIH, of the Asbestos Management Plan conducted by Wise Consulting & Training dated June 1999. Converse reviewed available surveillance information for review to determine compliance and whether any response actions including abatement and/or Operations and Maintenance (O&M) had been conducted.
Following AHERA re-inspection guidelines and utilizing EPA modeled forms, Converse reviewed the previous inspections and inspectors visited the subject site to re-evaluate the accessible identified ACBMs and/or presumed ACBMs. Additionally, the inspectors looked for the presence of any accessible thermal system insulation (TSI), surfacing material, or suspect miscellaneous materials that had not previously been identified.
!:\company files\2009\09-23635-01\VA-Reno AHERA Re-Inspection - 0 and M Upgrade\Bldg. 1 D
AHERA Asbestos Re-inspection
Clinic - Building 1 D
Pursuant to our scope of work, no bulk asbestos samples were obtained of TSI, surfacing and/or miscellaneous materials which had not been identified during the previous surveys. All previously unidentified suspect materials found during the re inspection are assumed to be ACBMs. Until the suspect ACBMs are sampled and laboratory analysis is performed to determine asbestos content these materials will be evaluated for management options and should be treated as ACBMs. Due to the policy of site specific evaluations, conducted in regards to renovation projects at the facility, it is cost effective to sample areas as to site specific renovation projects.
However, it must be noted that results of sampling, conducted in regards to the structure performed by Converse since the re-inspection conducted by IHI Environmental in 2001, have been included in Appendix D to simplify the accessibility of data for the client.
RESULTS
Assessment of Previously Identified ACBMs
Converse reviewed the previous inspection reports to identify all homogenous (i.e., like) ACBMs and/or presumed ACBMs. Those ACBMs known to be present within the structure consist of various floor tiles, black flooring mastic and sink coatings. These materials appeared to be in good condition and no change in material description and/or assessment category has been made in regards to the materials.
Assessment of Abated/Removed ACBMs
The records indicated that abatement had been conducted in regards to asbestos containing building materials (ACBMs) and/or presumed ACBMs. The areas of abatement including the materials abated and the quantity of those materials was identified as the following:
• The removal of approximately 1,000 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the areas designated as GD111, GD112, GD113 and GD115. This material was removed by PARC Specialty Contractors in July 2008.
• The removal of approximately 50 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the area designated as GD120. This material was removed by PARC Specialty Contractors in July 2008.
• The removal of approximately 60 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the area designated as t:lcompany files\2009\09-23635-01\VA-Reno AH ERA Re-Inspection - 0 & M Upgrade\Bldg. 1 D Page2

GD100. This material was removed by PARC Specialty Contractors in July 2008.
• The removal of approximately 5,600 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the Ground Floor in the areas designated as GD211, GD213 and GDC15. This material was removed by Allied Environmental, Inc. in December 2008.
• The removal of approximately 750 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the 1st Floor in the area designated as 1D162. This material was removed by PARC Specialty Contractors in November 2007.
• The removal of approximately 220 square feet of asbestos-containing VCT Floor Tile and/or Mastic located on the 1st Floor in the area designated as 1 D144.
(date of removal unknown)
Additional information regarding these materials is located in the attached AHERA Assessment Sheets and/or the Management Planner Information.
Assessment of Previously Unidentified ACBMs
In regards to homogenous materials located at the site, seven (7) additional suspect ACBMs were identified in addition to those which were previously identified. These materials consisted of the following:
• Vinyl Sheet Floor Covering located on the Ground Floor at the north end of the corridor designated as GD119.
• Base Cove/Mastic typical to all floors of the structure with the exception of the Ground Floor which has been sampled and determined to contain no asbestos.
• Drywall/Joint Compound located in various areas throughout the structure.
In regards to this material it must be presumed that asbestos may be present. If damage occurs and repairs can be made without the necessity of performing any cutting, grinding, sanding, or otherwise abrading the damaged surface in any fashion, such repairs can be conducted. If disturbance of the existing sheetrock materials is necessary and the area of repair is less than three (3) square feet or three (3) feet long, whichever is smaller, such repairs should only be conducted by properly trained O&M personnel using the appropriate Class Ill work methods.
t:lcompany liles\2009\09-23635-01\VA-Reno AH ERA Re-Inspection - 0 & M Upgrade\Bldg. 1 D Page 3
Clinic - Building 1 D
This exemption does not include or apply to the removal of any sheetrock materials exceeding O&M limitations. In those instances additional sampling is recommended to identify the presence of ACMs in regards to determining the actual Scope of Work, and the recommended procedures for removal by a certified Nevada licensed abatement contractor utilizing Class II removal methods.
• Ceramic Tile/Grout/Mortar (walls) located in various areas throughout the structure.
• Ceramic Tile/Grout/Mortar (floors) located in various areas throughout the structure.
• Exterior Wall Surfacing (Stucco) typical to the west portion of the south, east and north faces of the structure.
• Roofing Material/Mastics typical to the roof system of the structure.
These materials appeared to be in good condition and have been added to the O&M
· Program and Periodic Six-month Surveillance as presumed ACBMs. Until the presumed ACBMs are sampled and laboratory analysis is performed to determine asbestos content the materials will be evaluated for management options and should be treated asACBMs.
CONCLUSION
The AHERA re-inspection indicated that previously identified ACBMs had not changed condition. Also, no material descriptions and/or assessment categories have been changed and no additional locations have been added to previously identified ACBMs.
However, Converse identified seven (7) presumed ACBMs which had not been assessed during previous inspections and has added them for future assessment.
Converse recommends that the Sierra Nevada Health Care System of the Veterans Administration (VA) - loannis A. Lougaris Medical Center incorporate this report with the previous inspection reports and include this into the existing Management Plan. The VA-Reno "designated person" should retain a copy of this report for maintenance personnel use. Converse further recommends that the VA-Reno evaluate and consult this re-inspection before disturbance of an ACBM or suspect ACBM. Finally, the VA Reno should continue to conduct the Periodic Six-Month Surveillance inspections on this site and should inform staff that a re-inspection has occurred.
