About this file
This is a Solicitation, Offer, and Award (SF 1442) document for a Request for Proposal (RFP) issued by the Department of Veterans Affairs Network Contracting Office for a bolt replacement survey and replacement plan project at the Oklahoma City VA Health Care System.
The project requires contractors to survey and identify locations of ASTM 307 bolts and develop a replacement plan to replace them with ASTM A193 bolts at the Oklahoma City VA facility located at 921 Northeast 13th Street, Oklahoma City, Oklahoma 73104. The work scope includes locating and documenting all ASTM 307 bolts on high and medium pressure steam and hot water piping systems operating over 50 pounds per square inch, prioritizing replacement based on risk level, providing a report with bolt locations and quantities, and managing debris disposal and site cleanup. The contract period runs from April 15, 2026, through May 15, 2026 (30 days after Notice to Proceed). The project is set aside exclusively for Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) certified and listed in the SBA certification database. The cost range is between $25,000 and $100,000, with the NAICS code 541330 (Engineering Services). Proposals must be submitted via email to frances.basham@va.gov by April 9, 2026, at 10:00 A.M. Central Time. A non-mandatory site visit is scheduled for March 26, 2026, at 10:00 A.M. Evaluation will be based on past performance (two relevant federal, state, local government, or private contracts completed within five years), compliance requirements (OSHA/EPA violations, Experience Modification Rate, and LOS certification), and price. Award will be made to the responsible SDVOSB offeror whose offer provides the best value to the Government. The solicitation includes standard FAR clauses, construction wage rate requirements, Buy American requirements for construction materials, and limitations on subcontracting at 50 percent for services.

See SF 1442, Block 10 of this solicitation.
I.4A SELECTION CRITERIA/PROPOSAL PREPARATION INSTRUCTIONS
Proposal will be evaluated in accordance with RFO FAR 12.201 Simplified Acquisition Procedures
Award will be made based on the following factors:
Factor 1 - Past Performance Factor 2 – Compliance Requirements Factor 3 - Price Offerors are encouraged to provide their best offers with the past performance information requested for evaluation. The Government, based on the offered material presented, will award to the offeror that best demonstrates to be most advantageous for the Government.
Factor 1 - Past Performance: Offerors shall provide past performance information on two (2) federal, state, local government or private contracts of similar type, scope, size, and complexity. For purposes of this evaluation, a relevant project is further defined as completing construction modifications in a medical facility environment. Project shall have a minimum amount of $25,000.00 or more and completed within the past five years from the date of issuance of this RFQ. If a completed Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted with the proposal. The Government may also evaluate recent and relevant past performance, based upon sources available to the contracting officer that may include Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS) and Electronic Subcontract Reporting System (eSRS). IF THE OFFEROR CANNOT PROVIDE A CPARS FOR PROJECTS LISTED IN FACTOR 1, then the offeror shall submit Past Performance Questionnaires (PPQ).
Completed PPQs should be submitted directly from your client via email to Frankie Basham at Frances.Basham@va.gov. If you have any questions, please contact Christina Beeler-Blackburn at Frances.Basham@va.gov. If the offeror is unable to contact a client for a project(s) before closing date, the offeror should complete and submit with the proposal the first page of the PPQ, which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires.

If an offeror’s EMR is above 1.0, the offeror must submit a written explanation of the EMR from its insurance carrier on the insurance carrier’s letterhead, describing the reasons for the EMR and the anticipated date the EMR may be reduced to 1.0 or below.
· Adherence to clause 852.219-75: Offerors must provide documentation demonstrating compliance with clause 852.219-75
Failure to provide the required documentation for these factors may deem the offeror ineligible for award.
Factor 3 - Price: Offerors shall provide pricing as requested in the Schedule of Service identified within SF 1442
Basis of Evaluation: Past Performance will be evaluated in accordance with RFO FAR 12.203 and award will be made to the offeror presenting the best value to the Government.
The Government also reserves the right to award the lower priced offeror with lower past performance rating if the Contracting Officer (CO) determines the price difference does not merit awarding to the higher priced offeror. In that event, the CO shall make an integrated assessment of the best value award decision.
The Government will evaluate price based on the total price. Total price consists of the basic requirements. Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:
| • | Comparison of quoted prices received in response to the RFQ |
| • | Comparison of quoted prices with the IGCE. |
| • | Comparison of quoted prices with available historical information. |
