ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
Site Visit: A mandatory site-visit will be held at the Veterans Affairs Medical Center 500 Foothill Blvd. Salt Lake City, UT 84148 on July 14, 2019. The start time is 9:00am MDT. This will allow to hit the main areas in the morning. Then, a break for lunch and the afternoon will be allotted for question(s) or for more detailed look at spaces. Vendors will be required to enter through Wahlen Way (this is the only entrance on the campus) and follow signs to the main parking garage. The main entrance to the hospital is right in front of the parking garage. Erica Lichthardt and Courtney McArthur will escort the vendor(s) to GC07-1 (a space where vendors can leave their belongings while doing the walk through). Your point of contacts are Courtney McArthur with phone number 801-582-1565 X4085 or Erica Lichthardt with phone number 801-582-1565 X4077. Bring a copy of a contractor letter template (attached in beta.SAM portal).
Question from the Quoter: The Question submission period is the period of time between issuance of the solicitation and due date for quotes. This time period allows for an exchange of information between the VA and all potential quoters. All questions regarding this solicitation shall be sent in writing directly to the Contracting Officer by e-mail to susan.delacruz@va.gov. The last day to submit questions shall be July 14, 2020, 5:00 PM MDT July 15, 2020, 3:30pm.
Quote is due by July 21, 2020, 3:30pm MST August 4, 2020, 8:00pm MDT. Submit quote to the Contracting Officer Susan Dela Cruz at susan.delacruz@va.gov.
Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations.
The following factors shall be used to evaluate quotations:
(1) Equipment Requirements. Acceptable or Unacceptable Rating The vendor’s proposal demonstrates they meet the Wi-Fi and Virtual Bluetooth Low Energy (vBLE) cloud managed Wireless (Wi-Fi) and wayfinding solution (vBLE) technical requirements outlined in the Performance Work Statement.
(2) Technical capability. Acceptable or Unacceptable Rating The Vendor provides 24x7x365 technical support and on-site support within 24 hours i.e a requirement for emergency service. On-site support within maximum 24 hours – less time is better.
(3) Past Performance. Acceptable or Unacceptable Rating The Government shall make an objective assessment of the Offeror’s current/past performance submitted as part of their proposal. It will conduct a performance acceptable assessment based on aspects of offeror’s past performance that are recent and relevant which focuses on experience within the last three (3) years in the hospital settings that are comparable in size as VASLCHS or larger which contain ICUs, Operating Rooms, and other critical care areas. They have to be relevant, similar in complexity, and magnitude requirements of solicitation. Performance that are not recent, relevant, similar in complexity or magnitude will be evaluated with unacceptable performance in this area.
(4) Mandatory Site Visit. Acceptable or Unacceptable Rating.
(5) Price. The Government will evaluate the price by adding the total of all line item prices, including all options. The Total Evaluated Price will be that sum.
Provide a copy of itemized pricing for verification purposes in excel sheet.
E.9 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)
(a) Basis for Award. The Government will issue a contract to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price, and other factors considered.
(b)The following factors shall be used to evaluate quotations:

For the purposes of the award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 and 52.217-9 as follows:
The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but for no more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be at the same rates as those in effect under the contract at the time the option is exercised. The evaluation will therefore assume that the addition of the price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
(c) Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation.
