1.0 EXECUTIVE SUMMARY
Millennium Consulting Associates (Millennium) performed a limited, non-destructive asbestos and lead (XRF) survey, comprehensive finish schedules and abatement cost estimates of hazardous materials identified for the VA Medical Center in Sepulveda in accordance with Task Order No.
VA262-13-J-4558 and Contract No. GS-10F-0229U. The project is being conducted to determine the existence of Asbestos Containing Materials (ACM) and Lead Based Paint (LBP) in specified buildings. The final results of the survey are incorporated into this report providing a means for the VA to more efficiently plan for repairs, renovations and/or demolition projects.
The following buildings were included in the scope of the asbestos survey:
West Los Angeles Campus - Buildings 33, 44, 46, 113, 114, 115, 117, 156, 157, 158, 199, 205, 206, 208, 210, 211, 212, 213, 214, 215, 217, 218, 222, 224, 231, 236, 256, 257, 258, 259, 264, 295, 300, 301, 304, 305, 306, 329, 500, and 501. For all buildings, only the interior areas were included in this scope of work.

2.0 INTRODUCTION
Millennium conducted an asbestos survey for the VA West Los Angeles and Sepulveda campuses in accordance with Task Order No. VA262-13-J-4558 and Contract No. GS-10F-0229U from July 2013 through October 2013. This report is exclusive to the he VA West Los Angeles campus, located at 11301 Wilshire Blvd., Los Angeles, CA. The survey included multiple buildings throughout the campus. The final results of the survey will provide a means for the Los Angeles VA to more efficiently plan for repairs, renovations and demolition projects.
2.1 SCOPE OF WORK
The objective of this campus-wide limited asbestos and lead survey is to provide the following information:
1. Identify, document and sample suspect ACM;
2. Identify, document and sample for lead in paint;
3. Report, assess, and quantify all ACM and lead-based paint discovered during the survey;
4. Report, assess, and generally quantify all lead-based paint discovered during the survey; and
5. Provide comprehensive finish schedules and abatement cost estimates of asbestos-containing materials identified.
The asbestos and lead survey tasks included reviews of historical records, site visits, collection of representative samples, assessment of the condition of the materials, quantification of the asbestos-containing materials throughout the specified buildings, and a generalization of the lead components will be itemized. In accordance with the project requirements, samples were not taken if it rendered the material unstable, had the potential to cause leaking or other deterioration of the base material or if the sampling posed a hazard to those working in and around the building. The sampling of newly or recently renovated areas was omitted per the direction of the client.
2.2 SAMPLING METHODS, PROCEDURES AND PERSONNEL
A detailed sampling and analysis plan was prepared prior to the site survey. These sample collection and handling procedures were provided in the. In summary, the asbestos survey was performed using general procedures and protocols defined in EPA 40 CFR, part 763 and the lead survey was performed using general procedures and protocols as described in Chapter 7: Lead-Based Paint Inspection, “Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing,” U.S. Department of Housing and Urban Development, 1997 Revision.
The asbestos analyses were performed at an independent 3rd party accredited laboratory using the EPA 600/R-93/116 method. Additional confirmation analyses were not performed in accordance with the project requirements (e.g., point counting and transmission electron microscopy). All locations within the scope of work were physically inspected and categorized (i.e., AHERA and NESHAP categories), homogeneous area-by homogeneous area, to determine the presence of ACM.
Millennium conducted the lead surveys with an X-Ray Fluorescence (XRF) machine to identify painted surfaces which may contain lead and, therefore, require specific work practices during renovation activities. The sampling was not a comprehensive survey and, as such, was not intended to be compliant with U.S. Department of Housing and Urban Development (HUD) sampling requirements. Millennium performed the lead survey in general accordance with industry standards for renovation projects.
Millennium personnel held valid Cal/OSHA licenses for the respective type of work they performed. These asbestos specific licenses include: Certified Asbestos Consultants (CAC) and Site- Surveillance Technicians (SST). The lead specific licenses include: California Department of Public Health Inspector/Assessors and sampling technicians. The field staff and their respective certifications that performed site assessment activities are:
1. Mr. Jack McCubbin (CAC No. 93-0893),
2. Mr. Wesley Chase (ABIH Certification #: 10183, Exp. 6/1/2018; Certified Asbestos
Consultant #: 12-4846; CDPH-I/A #: 21068),
3. Mr. Kevin Milani (CSST No. 06-4035; CDPH-I/A No. 21310),
4. Ms. Nicoele Mitchell (CSST No. 11-4755), and
5. Ms. Laura Thomsen (CDPH-Sampling Technician & Trained I/A).
2.2.1 Review of Procedures: Asbestos Inspection, Bulk Sampling, Laboratory Information and Risk Assessment
The asbestos inspection and bulk sampling procedures were based on the guidelines established by the U.S. EPA in the Guidance for Controlling Asbestos Containing Materials in Buildings, Office of Pesticides and Toxic Substances, DOC #560/5-85-024 and 40 CFR Part 763, Asbestos Hazard Emergency Response Act (AHERA). Field information is organized according to the AHERA concept of Homogeneous Areas (HA). A HA is defined as a suspect material of similar age, appearance, function, and texture. Each material will be grouped together as a specific HA, sampled, and then assessed for condition. The AHERA guidelines represent an industry standard sampling protocol and as such was utilized during the inspection and sampling. For the purposes of this inspection, suspect ACM has been placed in three (3) material categories:
1. Thermal Systems Insulation (TSI);
2. Surfacing Materials; and
3. Miscellaneous Materials.
The locations within the buildings were physically inspected on a functional space-by-functional space and homogeneous area-by-homogeneous area basis, to determine the presence of ACM. Core samples of friable and non-friable suspect materials were collected by penetration of the suspect material to its substrate. The bulk samples collected were placed in individual sealed containers and labeled with unique sample identification numbers. Representative samples of each sampling area were then submitted to the laboratory to be analyzed for asbestos content.
Specifically, the inspection involved the following tasks:
1. Visual determination as to the extent of visible and accessible suspect materials and conditions of the material;
2. Collection and analyses of suspect building materials for asbestos content;
3. Determining condition (i.e., friable or non-friable) of suspect materials by physically touching, assessing, and placing the material into the correct ACM category (i.e., Physical Condition Assessment);
4. Quantification of the amount of suspect friable and non-friable materials along with their location;
5. Identification of all suspect materials sampled on the appropriate building floor plan diagram with the sample number; and
6. Preparation of a Chain-of-Custody form that accompanied the samples to the laboratory.
The U.S. EPA AHERA specifies that a physical assessment of all friable suspect material must be performed during the inspection. The suspect materials are assessed to determine if they pose a hazard, and if so, they are then ranked according to risk. The physical condition assessment consists of determining the condition of the ACM, the probable cause of damage, and the potential for future disturbance. AHERA lists seven categories in which to assess the current condition and potential for damage. These categories are as follows:
1. Damaged or Significantly Damaged Friable Thermal System Insulation;
2. Damaged Friable Surfacing Material;
3. Significantly Damaged Friable Surfacing Material;
4. Damaged or Significantly Damaged Friable Miscellaneous Material;
5. ACM with potential for damage;
6. ACM with the potential for significant damage; and
7. Any remaining Friable ACM or Assumed Friable ACM.
Additionally, as part of the assessment and categorization of materials, the ACMs are further defined by the current condition of the ACM, the potential for future damage, and a friability rating. These are defined as follows:
1. Current Condition of ACM: Intact, damaged or significantly damaged. A ranking system using a scale of 1-7 was used to evaluate the current condition of ACMs. A rank of "1" means the material is in "poor" condition and requires top priority for correcting the hazard risk. A result of "5" would indicate material in "fair" condition with moderate potential for future damage, which typically requires corrective actions. A rank of "7" indicates material in "good" condition with low potential for future damage; category 7 areas have a low hazard risk. The criteria for determining the damaged and significantly damaged conditions are as follows:
a. Damaged: The overall damage is up to ten percent (≤10%) and/or any localized damage is up to twenty-five percent (≤25%).
b. Significantly damaged: The overall damage is greater than ten percent (>10%) and/or any localized damage is greater than (>25%).
2. Potential for Future Damage of ACM: Another step in the assessment process is to determine the potential for future damage or deterioration for material classified as good or fair. The potential for future damage was classified as High, Moderate, or Low. There are many factors to consider in determining whether or not there is potential for future damage. Some of these factors may include potential for physical contact and the influence of environmental factors such as vibration, air erosion, the likelihood of water damage, etc.
3. Friability Rating: All material were given a friability rating and classified as either Friable ACM or Non-Friable ACM. Friable ACM is defined as any material that contains more than one percent (>1%) asbestos and can be crumbled, pulverized, or reduced to powder by hand pressure. It is a description of risk and refers to a material’s likeliness to release airborne fibers.
Chain-of-Custody procedures and records provide a means of tracing each sample from the time of collection through the shipment and final analyses. Proper Chain-of-Custody procedures also provide a written record of all persons handling the samples. The information necessary to relate sample locations for reporting purposes is documented with each sample. This information is essential to proper completion of a sample results report and typically includes client and facility information; unique sample identification number; date and time sampled; sampler name and signature; sample location including the room/area; and field sketches of sample locations.
All sample analyses were performed by AmeriSci, Inc., Carson, CA, a laboratory which is certified by the State of California Environmental Laboratory Accreditation Program (CA ELAP No. 2322), the U.S. Department of Commerce, National Institute of Standards and Technology (NIST) Proficiency Analytical Testing (PAT No. 100530) program, and is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP No. 200346-0) for bulk asbestos analyses. The laboratory will be requested to retain the samples for at least ninety (90) days in the event re-analyses of samples are required. All disposal of samples will be compliant with established Federal, state, and local regulations.
2.2.2 Asbestos Regulatory Overview
Construction materials containing asbestos greater than 1 percent are defined as an ACM and are regulated under both federal and state regulations. Construction materials containing asbestos greater than 0.1% are defined as an Asbestos-Containing Construction Material (ACCM) and are regulated by the State of California. Cal/OSHA regulates the removal of both ACM and ACCM.
Please refer to Title 8§1529-Asbestos for the regulatory requirements associated with working with both ACM and ACCM. Additionally, refer to §1529(r)-Report of Use and Asbestos-related Work Registration for the registration requirement of contractors involved in asbestos-related work involving over 100 square feet of ACCM/ACM. In instances where a material contains asbestos in concentrations below the ACCM regulatory threshold, the employer is required to comply with Cal/OSHA 5194-Hazard Communication in addition to pertinent sections of §1529-Asbestos.
In California, ACMs that are >1% and friable or will become friable during abatement are classified as a California-Hazardous Waste, and require special handling, packaging and disposal.
2.2.3 Lead Paint Sampling and Analysis
A NITON (Model No. XLp 303A), a hand-held, battery operated energy dispersive x-ray fluorescence (XRF) analyzer was used for the lead paint survey. The XRF is utilized for the detection and quantification of elements ranging from phosphorus (atomic number 15) through uranium (atomic number 92). A positive classification indicates that lead is present on the painted surface at or above the California Department of Public Health (CDPH) standard of 1.0 mg/cm2. NOTE: Millennium used the CDPH definition of lead-based paint versus Los Angeles County’s definition, which is 0.7 mg/cm2.
The XRF measurement locations are defined by the following:
Wall A is the front wall or the wall that parallels the street that gives the site its address. Walls B, C and D go clockwise around the building or room from wall A. The C wall is the rear wall. Each room has a wall A, B, C and D and each closet has an A, B, C and D wall.
2.2.4 Lead Regulatory Overview
Worker Protection and Waste Definitions of Lead (in paint and construction materials)
Other Regulatory Definitions of lead-containing materials are detailed in 8 CCR and 22 CCR and CFR title 40 regulations. Cal/OSHA 1532.1-Lead regulates the removal of materials with detectable levels of lead. Please refer to §1532.1-Lead for the regulatory requirements associated with working with lead-containing materials.
It is important to understand that Cal/OSHA does not give a regulatory definition of a “lead-containing material.” Cal/OSHA and Federal OSHA are concerned with “an employee occupationally exposed to lead.” This is understood to mean material disturbed during construction work containing lead in any amount (i.e., lead-containing paint and lead-based paint) is covered under the lead in construction standard. Additionally, Federal OSHA has determined that the uses of XRF data and/or bulk sampling data (e.g., paint chips) are not acceptable for predicting employee exposures to lead. This fact means that contractors cannot use XRF data, paint chip data or bulk sample data as a surrogate for employee exposures during construction work (or the bidding process) as defined in 8 CCR 1532.1(a). The two OSHA interpretation letters below should be reviewed. Again, in summary they state, the burden of proof is on the employer in regards to employee exposures to lead in construction work and not the reliance on XRF data, bulk sampling data or paint chip sampling data. Current California and Federal regulations do mandate that generators determine if a waste is hazardous or non-hazardous by testing representative samples of the waste. The total lead by Total Threshold Limit Concentration (TTLC), California WET-method Soluble Threshold Limit Concentration (STLC), and Toxicity Characteristic Leaching Procedure (TCLP) analyses should be performed to characterize each waste stream as Federal Resource Conservation Recovery Act (RCRA) hazardous waste, California hazardous waste, non-hazardous waste, or as construction debris. The waste stream must be handled as RCRA environmentally hazardous waste if TCLP lead levels exceed 5.0 milligrams per liter (mg/l), or as California hazardous waste if TTLC lead exceeds 1,000 milligrams per kilogram (mg/kg), and/or STLC lead exceeds 5.0 mg/l, respectively. By calculation, if a sample analyzed for lead by TTLC is found to contain less than 50 mg/kg, then the waste stream represented by the sample result is non-hazardous by definition (a completely soluble waste at this concentration would produce a TCLP lead concentration of less than 5.0 mg/l). Similarly, total lead less than 50 mg/kg will generally produce an STLC lead concentration of less than 5.0 mg/l.
3.0 SITE HISTORY AND BUILDING SUMMARY
The United States has the most comprehensive system of assistance for veterans of any nation in the world. The VA health care system has grown from 54 hospitals in 1930, to include 152 hospitals;
800 community based outpatient clinics; 126 nursing home care units; and 35 domiciliaries. VA health care facilities provide a broad spectrum of medical, surgical, and rehabilitative care. This project includes the VA Medical Centers located at West Los Angeles and Sepulveda.
The VA West Los Angeles campus has conducted considerable remodeling of various buildings and areas, particularly in the main hospital (i.e., Building 500, ca. 1976). The detailed list of buildings and areas included in the project are as follows:

4.0 SURVEY RESULTS
The objective of this campus-wide limited asbestos and lead survey is to (1) Identify, document and sample suspect ACM; (2) Identify, document and sample for lead in paint; and (3) Report, assess, and quantify all ACM and lead-based paint discovered during the survey; (4) Report, assess, and generally quantify all lead-based paint discovered during the survey; and (5) Provide comprehensive finish schedules and abatement cost estimates of asbestos-containing materials identified.
ACM Data Results
A total of six hundred ninety-nine (699) bulk samples were collected within the buildings and then analyzed on a layer-by-layer basis using polarized light microscopy (PLM). The following sections lists the positive building materials identified, sampled, and analyzed during this investigation. The Summary of Associated Removal Estimates by Building is presented in Table 1, the Survey Summary of Positive ACM Result and Associated Removal Estimate is presented in Table 2, and the Survey Summary of Lead-Based Paint Result and Associated Removal Estimate is presented in Table
3. The Materials results by location of each building is presented in the Finish Schedule, Appendix C. Please refer to Appendix A - the Laboratory Reports in Appendix B for a full listing of all materials sampled. Every reasonable effort was made to access all areas within the scope of work.
The Risk Assessment for each identified ACM and the NESHAP Category is presented within the Management Plan, a separate document
NOTE: Millennium used historical survey information, which were provided by the VA, in order to perform a data gap assessment and assist us with our sampling and analysis plan. Millennium’s limited survey report should not be a stand-alone document as the historical survey information should be referenced as well. Please refer to Appendix C for the previous survey information.
Lead Paint Data Results
A total of one thousand three hundred three (1303) XRF readings were collected at various locations within buildings, not including calibrations and standardizations. The analytical results from XRF data of the lead samples indicate that three hundred seventy (370) readings registered at or above 1.0 mg/cm2. Based on the XRF data, some of the readings are considered to be either lead-containing materials or lead-based. A complete breakdown of the surfaces sampled are provided in Appendix B.
BUILDING 500 – MAIN HOSPITAL
Building No. 500 is the main hospital and was built in 1976. This building contains seven floors and is 900,000 ft2 in size. The interior includes a mix of drywall and plaster walls. The exterior is composed of concrete walls with a built-up membrane parapet roof. The HVAC system is a forced air system with the return air located in the plenum space and the fresh air intake located on the roof.
This building has undergone numerous renovations of select areas and systems (e.g., various floor tile and mastic removal projects, ceramic tile removal in the restrooms, and loose/flaky lead paint.)
since its construction.

The total ACM/Lead removal costs for building 500 are $13,016,830. The total asbestos removal cost is $12,082,780 and lead-based paint removal costs for building 500 are $934,050. Please refer to Tables 1-3 included herein.
BUILDING 501 – POWER PLANT BUILDING
Building No. 501 is the Power Plant building and was built in 1976. The building has two floors and is 30,000 ft2 in size. The interior includes a mix of drywall, plaster, CMU and concrete walls with painted concrete and drop-in ceiling panels. The interior flooring finishes include 12”x12” & 18”x18” vinyl floor tiles, ceramic tiles and concrete. The exterior is composed of a slab-on grade foundation with a stucco exterior walls and a flat roof. The HVAC system is comprised of a forced air system with at least one air handling unit located on the ground adjacent to the building.
Historical Data:
No previous ACM or lead paint data was provided.
Millennium’s Data:
ACM was detected in the wallboard joint compound, ceiling tile mastic and 12” floor tile.
Millennium discovered lead-based paint in the concrete walls, ceramic wall tiles and the yellow metal post.
The details of location, condition, color, substrate analytical results are provided in the appendix of this report.

The total ACM/Lead removal costs for building 501 are $5,580. The total asbestos removal cost is $2,880 and lead-based paint removal costs for building 501 are $2,770. Please refer to Tables 1-3 included herein.
5.0 COST ESTIMATE METHODOLOGY & RECOMMENDATIONS
Cost Estimates
The unit prices are based on current market conditions in the greater Los Angeles area at the time of the survey. The prices reflect discrete tasks without the benefit of economy of scale that might be realized in an actual construction bidding environment. The estimates are for planning purposes only. Should the buildings be scheduled for abatement, additional investigation - including in areas that were either not accessible or would require destructive or invasive methods - will be required.
Actual costs will vary; and are dependent upon many factors, including but not limited to project magnitude, project schedule, current bidding environment and project duration.
Each unit price is a fully loaded value which is comprised of 70% labor, 20% materials/disposal and 10% equipment.
Recommendations
Based on the findings and conclusions from the survey, Millennium presents the following recommendations:
1. It will be necessary to comply with all applicable provisions of local, EPA, OSHA, Cal/OSHA, and South Coast Air Quality Management District (SCAQMD) regulations during any removal, demolition, or repair activities that may disturb the asbestos-containing materials and/or asbestos-containing construction materials.
2. Caution should be exercised when entering asbestos-contaminated buildings, such as Building 156 and 157. These buildings should only be accessed by personnel properly trained and certified to don personal protective equipment including respiratory protection..
3. Areas not accessed or surveyed during this survey should be investigated prior to any planned renovations.
4. All VA Employees relying on this document as part of an Asbestos/Lead Hazard Management Program should have proper training in the recognition, evaluation and control of hazardous materials.
6.0 LIMITING CONDITIONS
Millennium Consulting conducted the survey from October and November of 2013, in accordance with industry standards in existence at the time of the project. The conclusions and recommendations presented in this report are based on the applicable standards of our profession at the time this report was prepared. Copies of this report are furnished to provide factual data that were gathered and summarized in the report.
The scope of this investigation was limited to accessible interior building finishes using non-destructive methods. This document is not intended to satisfy NESHAPs pre-renovation survey requirements. Additional asbestos containing construction materials may be located in chases, cavities, above/behind secondary components (i.e. ceiling systems) and underneath carpet of similar floor finishes.
The analysis and recommendations submitted in this report are based in part on the reliance of data obtained from other entities. However, the nature and extent of variations between the sampling locations may not become evident until the planned asbestos survey commence. This report has been prepared for the exclusive use of the Client for specific application to the survey performed on the property.
