About this file

This is a Limited Asbestos Survey Report dated November 3, 2016, prepared by HGS Engineering Inc. for the Department of Veterans Affairs (VA) Medical Center in Big Spring, Texas, specifically for Building 2 (Warehouse Boiler Plant).

The survey was conducted to identify asbestos-containing materials (ACMs) and asbestos-containing building materials (ACBMs) that may be disturbed during future renovations. HGS performed bulk sampling of twelve materials in September 2016, including drywall with texture, clear carpet mastic, yellow carpet mastic, and covebase mastic from newly renovated areas. All twelve samples tested returned "None Detected" for asbestos using Polarized Light Microscopy/Dispersion Staining (PLM/DS) analysis conducted by Steve Moody Laboratories Inc. (TDSHS Laboratory License No. 300084, NVLAP accredited). The survey was limited to accessible interior areas; the inspectors were TDSHS-licensed asbestos consultants Lonnie Lyro (License No. 105763) and Trina Mullen (License No. 10-5687). The report reviewed a previous 2011 survey by Dougherty Sprague Environmental that identified several ACBMs in Building 2, including black mastic under yellow vinyl composite tile in the Boiler Plant Offices and Locker Room, black under-sink coating in the Water Treatment Room, white mastic on thermal system insulation, and window caulking. HGS observations confirmed these previously identified ACBMs remain in place and recommended that any materials to be disturbed during renovation be assumed to contain asbestos until proven otherwise through sampling and laboratory analysis. Materials not disturbed can be disposed of as construction debris; abatement of identified ACMs must be performed by TDSHS-licensed contractors in compliance with EPA NESHAP standards and OSHA regulations.

RE: Limited Asbestos Survey Veterans Affairs (VA) Medical Center 300 West Veterans Boulevard, Big Spring, Texas 79720

Building 2

Dear Mr. Moore:

HGS Engineering Inc. (HGS) was retained by Department of Veterans Affairs Contracts 00504 Engineering Services – Big Springs, Texas Location to perform a limited interior asbestos survey of the building materials at Building 2, and a review of a previous survey conducted by Dougherty Sprague Environmental, Inc. (dse) in June 2011. This investigation included a visual inspection and physical survey to identify suspect asbestos-containing materials (ACMs) or asbestos-containing building materials (ACBMs) and evaluate their general condition. The scope of work for this assessment included the sampling and laboratory analysis of suspect ACMs that may be disturbed during future renovations.

dse ACM SURVEY JUNE 2011 (previous survey)

According to dse, “Comprehensive asbestos inspections of the facility were performed in 1995 by Encon International (Encon) and in 2004 by Asset Group, Inc. (Asset Group). In addition, several consultants have performed limited inspections of building areas scheduled for renovation as early as 1986 and as recently as 2008. dse reviewed all of the available asbestos inspection reports prior to performing the re-inspection of the facility to avoid re-sampling homogeneous areas that were adequately sampled during earlier inspections and shown to be non-ACBMs. Moreover, the classification of homogeneous areas shown to be ACBMs during earlier inspections cannot be changed by re-sampling unless the homogeneous areas are redefined or the previous sampling results are in question because of poor documentation. In general, the 1995 and 2004 comprehensive inspections were well documented; however, the limited inspections were poorly documented with minimal supporting data such as drawings, photos and laboratory reports.”

Building 2 – Warehouse Boiler Plant

According to dse:

ACBM Black Mastic under Yellow VCT identified as ACBM in the Boiler Plant Offices and Locker Room.

Black under-sink coating was identified as ACBM on sinks in the Water Treatment Room.

One sample of white mastic on yellow painted TSI was identified as ACBM.

Window caulking on exterior of old and new windows was identified as ACBM.

Asbestos-containing Transite panels and black asbestos-containing mastic may be present behind steam radiators.

SITE DESCRIPTION AND HGS ACM SURVEY, SEPTEMBER 2016

The survey was limited to the interior of Building 2. According to the RFQ-VA258-16-Q-0434 and a Questions and Answer submittal, per Eddie Henderson, “The VA is not concerned about ACM exterior sampling of concrete, brick, mortar, pavement and walkways, nor ceramic tiles, and only Building 1 roof is to be sampled”

HGS’s survey was limited to the accessible suspect building materials inside Building 2 and materials not sampled previously by dse in June 2011 or materials that have been installed since 2011. HGS identified the following materials that were not previously sampled and may be impacted by future renovation activities during this assessment:

Drywall and Texture in Supply Office (new build-out since 2011);

Clear Carpet Mastic in Supply Office (new build-out since 2011);

Yellow Carpet Mastic in Supervisor Office 29; and Covebase and Mastic in Offices.

HGS ACM SURVEY

Bulk sampling was conducted in general accordance with procedures outlined in the Texas Department of State Health Services (TDSHS) Texas Asbestos Health Protection Rules (TAHPR), the U.S. Environmental Protection Agency (EPA) National Emissions Standards for Hazardous Air Pollutants (NESHAP) and the EPA guidance document entitled Guidance for Controlling Asbestos-Containing Materials in Buildings (Document No. 560/5-85/024). The asbestos inspection was performed by a TDSHS-licensed asbestos inspectors Mr. Lonnie Lyro (Individual Asbestos Consultant License No. 105763) and Ms. Trina Mullen (Individual Asbestos Consultant License No. 10-5687) on September 14, 2016.

HGS collected twelve (12) bulk samples of suspect ACMs from the interior of Building 2. Bulk samples of suspect building materials were analyzed by Steve Moody Laboratories Inc., in Carrollton, Texas (TDSHS Laboratory License No. 300084). Steve Moody Laboratories is accredited with the EPA Interim Asbestos Bulk Sample Analysis Quality Assurance Program and the National Voluntary Laboratory Accreditation Program (NVLAP No. 102056-0). The PLM/DS analytical method is modeled after U.S. EPA Publication EPA/600/R-93/116: Test Method for the Determination of Asbestos in Bulk Materials, July 1993. If a material is identified as containing greater than 1% asbestos, it is considered to be an ACM. The Bulk Sample Analysis Reports, Chain of Custody, and applicable licenses/certifications, are attached. The asbestos report dated September 23, 2016 can be found in the attachments.

LIMITATIONS

This report is not to be utilized as a bidding document or as a project specification document since it does not have all the components required to serve as an Asbestos Project Design document or an Abatement Workplan.

Our professional services have been performed, our findings obtained, and our conclusions prepared in accordance with customary principles and practices in the fields of environmental science and engineering.

This statement is in lieu of other statements either expressed or implied. This report does not warrant against future operations or conditions, nor does it warrant against operations or conditions present of a type or at a location not investigated. This asbestos survey was limited to the accessible areas of the buildings on the West Texas VA Medical Center facility located at 300 Veterans Blvd., Big Spring, Texas. No effort was made to open spaces such as pipechases, crawl spaces, walls and attics that did not have access doors or hatches. In addition, no building components such as HVAC equipment, electrical equipment, pipes or plumbing fixtures that would require specially trained workers to remove or reinstall were disturbed. Locked and/or occupied rooms were not inspected if similar “functional spaces” were accessible.

This report is certified to the VA in Big Springs, Texas. The scope of services performed in execution of this evaluation may not be appropriate to satisfy the needs of other users, and use or re-use of this document or the findings, conclusions, or recommendations is at the risk of said user.

CONCLUSIONS

All materials included as part of HGS survey on September 15, 2016 were reported as “None Detected” for the presence of asbestos.

According to the 2011 dse asbestos-containing materials survey:

ACBM Black Mastic under Yellow VCT identified as ACBM in the Boiler Plant Offices and Locker Room;

Black under-sink coating was identified as ACBM on sinks in the Water Treatment Room;

One sample of white mastic on yellow painted TSI was identified as ACBM;

Window caulking on exterior of old and new windows was identified as ACBM.

Asbestos-containing Transite panels and black asbestos-containing mastic may be present behind steam radiators;

Black Waterproofing has been identified on the interior of exterior walls;

ACBM Thermal System Insulation (TSI) and white or black seam sealant may be present on all pipes within walls, ceiling, attics and crawlspaces.

HGS made the following observations in regard to ACBMs within Building 2 during the 2016 Survey:

Asbestos-containing VCT and associated mastic was removed/abated from Locker Staff 370;

Asbestos-containing VCT was in place under non-ACM sheet flooring within Office 31 and 32;

Asbestos-containing VCT was in place under carpet in Office 330;

Asbestos-containing VCT is present within Water Test Room 37B;

Black sink undercoating was observed on sinks in the Water Treatment Room;

Pipe insulation mastic (white) was observed above the Boiler Room Restroom;

Window caulking was observed on exterior windows;

Transite panels and black waterproofing behind radiators, black waterproofing on the interior of exterior walls, and TSI and black or white seam sealant within chases, walls, ceiling and crawlspaces are considered concealed materials and shall be assumed to contain asbestos until otherwise sampled and proven a non-ACM material; and

Roofing materials have not been sampled and shall be assumed to contain asbestos until otherwise sampled and proven a non-ACM material.

RECOMMENDATIONS

All materials previously sampled and determined not to contain asbestos, and those materials sampled during the 2016 survey (Drywall with Texture, Clear Carpet Mastic, Yellow Carpet Mastic, and Covebase

Mastic), which were determined not to contain asbestos, are not regulated and can be removed and disposed of as construction debris. If any additional materials are to be disturbed during renovation activities, these materials must be assumed to contain asbestos until sampling and analysis proves otherwise. A comprehensive list of the sampled building materials can be found within the Asbestos Management Plan.

Prior to renovation/demolition, any identified ACM that could/may be disturbed must be properly removed by a TDSHS licensed asbestos abatement contractor. The licensed abatement contractor should remove the materials in compliance with the designed site specific scope of work, EPA NESHAP asbestos standards, TDSHS TAHPR, and OSHA 29 CFR 1926.1101. During abatement related activities the owner is required to have a TDSHS licensed Project Manager/Air Monitoring Technician (working under a licensed Asbestos Consulting Agency) on site, to provide air monitoring and documentation verifying applicable regulations are being followed.

Subcontractors and employees working within the structures at the site should be made aware of the locations of the ACM and the possibility of concealed ACMs that could be found during renovation/demolition activities.

Additional ACM and ACBMs may be present at the site in inaccessible or concealed spaces. These spaces include, but are not limited to, pipe chases, spaces between wall/ceiling/door/floor cavities, interior of mechanical components such as boiler cavities, interior ducts, beneath foundation pads, etc. If future maintenance/renovation/demolition activities make these areas accessible, HGS recommends that a thorough assessment of these spaces be conducted at that time to identify and confirm the presence or absence of additional ACMs. Until then, all such unidentified materials should be treated as assumed ACM in accordance with 40 CFR 76.
