DUST CONTROL PLAN NOTES

1. THE VAMC INFECTION CONTROL GROUP HAS CONFIRMED THE DESIGNATION FOR THIS

PROJECT IS LEVEL IV FOR THE CLASS OF PRECAUTIONS OR LEVEL OF INFECTION CONTROL

ACTIVITIES. HARD SURFACE BARRIERS SHALL BE USED EXCEPT IN THE AREA INDICATED ON

THE PHASING PLAN WITH PLASTIC BARRIERS (MINIMUM 4MIL) TO SEAL AREA FROM NON-WORK

AREA AS LONG AS THE SPRINKLER SYSTEM IS INTACT AND ACTIVE.

2. PROVIDE STICKY MATS AT THE ICRA BARRIER ENTRANCE AND CHANGE DAILY OR MORE

OFTEN AS NEEDED TO PREVENT THE SPREAD OF DUST AND DEBRIS INTO ADJACENT CLEAN

SPACE.

INTERNAL DEMOLITION AND CONSTRUCTION ACTIVITIES DUST AND DEBRIS CONTROL

1. USE EXTERIORS DOORS AS PRIMARY CONSTRUCTION ACCESS.

2. TRAFFIC CONTROL: DESIGNATED ENTRY AND EXIT PROCEDURES SHALL BE DEFINED. EGRESS

PATHS SHALL BE FREE OF DEBRIS; DESIGNATED ELEVATORS SHALL BE USED DURING

SCHEDULED TIMES; AND ONLY AUTHORIZED PERSONNEL SHALL BE ALLOWED TO ENTER

THE CONSTRUCTION ZONE. SIGNAGE SHALL DIRECT PEDESTRIAN TRAFFIC AWAY FROM THE

CONSTRUCTION AREA AND MATERIALS.

3. CONTRACTOR SHALL USE TWO CARTS TO TRANSFER CONSTRUCTION MATERIAL IN AND OUT

OF THE PROJECT - A CLEAN CART SHALL BE USED; THE NOT-SO-CLEAN CART SHALL BE USED

BETWEEN THE MAIN ENTRY AND THE CONSTRUCTION STAGING/PREP AREA.

4. DEMOLITION DEBRIS: DEBRIS SHALL BE REMOVED IN CARTS WITH TIGHTLY FITTED COVERS, USING DESIGNATED TRAFFIC ROUTES. EFFORTS SHALL BE MADE TO MINIMIZE USE OF

ELEVATORS WITH AN EMPHASIS ON TRANSPORT DURING THE LOWEST PERIOD OF ACTIVITY.

DEBRIS SHALL BE REMOVED DAILY AND AT TIMES SPECIFIED BY THE VAMC. IF CHUTES ARE

USED TO DIRECT DEBRIS OUTSIDE, HIGH EFFICIENCY PARTICULATE AIR (HEPA) FILTERED

NEGATIVE AIR MACHINES SHALL BE USED, AND THE CHUTE OPENING SHALL BE SEALED WHEN

NOT IN USE. FILTERS SHALL BE BAGGED AND SEALED BEFORE BEING TRANSPORTED OUT OF

THE CONSTRUCTION AREA. THE CONTRACTOR SHALL NOT HAUL DEBRIS THROUGH PATIENT-

CARE AREAS WITHOUT PRIOR APPROVAL OF THE COR.

5. EXTERIOR WINDOWS: WINDOWS SHALL BE SEALED TO MINIMIZE INFILTRATION FROM ANY

ADJACENT EXCAVATION DEBRIS.

6. ALL FURNISHINGS AND EQUIPMENT TO REMAIN IN THE IMPACTED ROOMS SHALL BE COVERED

AND PROTECTED PRIOR STARTING EACH CONSTRUCTION PHASE.

VENTILATION AND ENVIRONMENTAL CONTROLS

1. AIR SYSTEM FLOW: VERIFY WITH THE COR WHETHER CONSTRUCTION AREA USES

FRESH/OUTSIDE OR RE-CIRCULATED AIR; FILTERS SHALL BE ADDED OR RETURN VENTS

COVERED AS NEEDED WITH FILTER MATERIAL OR PLASTIC. AIR MUST FLOW FROM CLEAN TO

DIRTY AREAS.

2. NEGATIVE AIR PRESSURE: THE AIR WITHIN THE CONSTRUCTION AREA SHALL BE NEGATIVE

WITH RESPECT TO SURROUNDING AREAS AND WITH NO DISRUPTION OF AIR SYSTEMS OF

ADJACENT AREAS. USE OF THE NEGATIVE AIR PRESSURE SYSTEM WITHIN THE ENCLOSURE

TO REMOVE DUST SHALL PASS AIR THROUGH AN INDUSTRIAL GRADE, PORTABLE HEPA FILTER

CAPABLE OF FILTRATION RATES OF 300-800 CUBIC FEET PER MINUTE (FT3/MIN), OR EXHAUST AIR DIRECTLY TO THE OUTSIDE IF APPROVED BY VAMC. IF EXHAUST SHALL BE TIED INTO A RE-

CIRCULATED AIR SYSTEM, A PRE-FILTER AND HEPA FILTER SHALL BE USED BEFORE EXHAUST

TO PREVENT CONTAMINATION OF THE DUCTS.

3. FOR EXHAUST DUCT ROUTING THRU CORRIDOR: INSTALL HARD DUCT WITH NON-FLANGE

HANGER HUNG FROM CEILING ABOVE, BOTTOM OF DUCT SHALL BE A MINIMUM OF 7'-2" A.F.F.

4. ADJACENT AREAS: THE STATUS OF SEALED PENETRATIONS AND INTACT CEILING SHALL BE

VERIFIED DAILY.

5. AIR EXCHANGE RATES AND PRESSURE RELATIONSHIPS: VAMC AND CONTRACTOR SHALL

VERIFY AND MAINTAIN PROPER RATES IN CRITICAL AREAS NEAR CONSTRUCTION ACTIVITY

AND ENSURE AIR IS NOT BEING RE-CIRCULATED WITHOUT FILTRATION FROM THE

CONSTRUCTION AREA ELSEWHERE. PROVIDE AIR PRESSURE TESTING THROUGHOUT THE

PROJECT IN THE FREQUENCY AS IDENTIFIED BY THE COR.

CONTAMINATION OF PATIENT ROOMS, SUPPLIES, EQUIPMENT AND RELATED AREAS

1. WORKSITE CLOTHING: CONTRACTOR PERSONNEL CLOTHING SHALL BE FREE OF LOOSE SOIL

AND DEBRIS BEFORE LEAVING THE CONSTRUCTION AREA. IF PROTECTIVE APPAREL IS NOT

WORN (E.G., COVERALLS, FOOTGEAR AND HEADGEAR) A HEPA-FILTERED VACUUM SHALL BE

USED TO REMOVE DUST FROM CLOTHING BEFORE LEAVING THE BARRICADE. IF PROTECTIVE

APPAREL IS UTILIZED, THE CONTRACTOR SHALL CONSTRUCT A SPACE OR ANTEROOM FOR

CHANGING CLOTHING AND STORING EQUIPMENT (DESIGNATED AREA. ALL EQUIPMENT,

TOOLS, TOOL CARTS, AND MATERIALS TRANSPORTED THROUGH OCCUPIED AREAS SHALL BE

MADE FREE FROM DUST AND MOISTURE BY VACUUMING AND WET WIPING BEFORE THEIR

REMOVAL FROM THE CONSTRUCTION ZONE OR WORK AREA.

2. POST CONSTRUCTION: DO NOT REMOVE BARRIERS AND KEEP ALL HVAC ISOLATION UNTIL

CONTRACTOR HAS CLEANED CONSTRUCTION SITE, VA EMS STAFF HAS DONE A TERMINAL

CLEANING, AND VA INFECTIOUS DISEASE CONTROL HAS INSPECTED AND APPROVED THE

CLEANING.

During Construction Project Upon Completion of Project

1. Includes all activities required by Class III

2. Obtain Infection Control Permit from Hospital Safety Officer or Facilities Management

Maintenance and Engineering department before construction begins.

3. Construct and complete critical barriers meeting NFPA 241 requirements. Barriers must extend to the ceiling or if ceiling tile is removed, to the deck above.

4. All (plastic or hard) barrier construction activities must be completed in a manner that prevents dust release. Plastic barriers must be effectively affixed to ground and ceiling and secure from movement or damage. Apply tape that will not leave a residue to seal gaps between barriers, ceiling or floor.

5. Seal all penetrations in containment barriers, including floors and ceiling, using approved materials (UL schedule firestop if applicable for barrier type).

6. Containment units or environmental containment units (ECUs) approved for Class IV precautions in small areas totally contained by the unit and that has HEPA-filtered exhaust air.

7. Remove or isolate return air diffusers to avoid dust entering the HVAC system.

8. Remove or isolate the supply air diffusers to avoid positive pressurization of the space.

9. Negative airflow pattern must be maintained from the entry point to the anteroom and into the construction area. The airflow must cascade from outside to inside the construction area. The entire construction area must remain negatively pressurized.

10. Maintain negative pressurization of the entire workspace by use of HEPA exhaust air systems directed outdoors. Exhaust discharged directly to the outdoors that is 25 feet or greater from entrances, air intakes and windows does not require HEPA-filtered air.

11. If exhaust is directed indoors, then the system must be HEPA filtered. Prior to start of work, HEPA filtration must be verified by particulate measurement as no less than 99.97% efficiency and must not alter or change airflow/pressure relationships in other areas.

12. Exhaust into shared or recirculating HVAC systems, or other shared exhaust systems (e.g., bathroom exhaust) is not acceptable.

13. Install device (e.g., magnehelic, manometer, or digital monitoring) on exterior of work containment to continually monitor negative pressurization. The “ball in the wall” or similar apparatus are not acceptable.

14. Contain all trash and debris in the work area.

15. Nonporous/smooth and cleanable containers (with a hard lid) must be used to transport trash and debris from the construction areas. These containers must be damp-wiped cleaned and free of visible dust/debris before leaving the contained work area.

16. Worker clothing must be clean and free of visible dust before leaving the work area.

HEPA vacuuming of clothing or use of cover suites is acceptable.

17. Workers must wear shoe covers prior to entry into the work area. Shoe covers must be changed prior to exiting the anteroom to the occupied space (non-work area). Damaged shoe covers must be immediately changed.

18. Install a sticky (dust collection) mat at entrance of contained work area based on facility policy. Sticky mats must be changed routinely and when visibly soiled.

19. Consider collection of particulate data during work to monitor and ensure that contaminates do not enter the occupied spaces. Routine collection of particulate samples may be used to verify HEPA filtration efficiencies.

9. Remove or isolate the supply air diffusers to avoid positive pressurization of the space.

10. Nonporous/smooth and cleanable containers (with a hard lid) must be used to transport trash and debris from the construction areas. These containers must be damp-wiped cleaned and free of visible dust/debris before leaving the contained work area.

11. Install a sticky (dust collection) mat at entrance of contained work area based on facility policy. Sticky mats must be changed routinely and when visibly soiled.

12. Maintain clean surroundings when area is not contained by damp mopping or HEPA vacuuming surfaces.

13. Clean construction area at the end of each shift by damp mopping or HEPA Vacuum.

For Type C activities, follow procedures for Class IV.

BURIED STRUCTURES, UTILITIES, ETC., AND SHALL IMMEDIATELY NOTIFY THE COR

OF ANY SITE CONDITIONS NOT REFLECTED ON THE DRAWINGS OR DIFFERENT

FROM MAXIMUM OR MINIMUM DIMENSIONS INDICATED, INCLUDING CONFLICT IN

GRADES, ADVERSE SOIL CONDITIONS, GROUND WATER PRESENT, DEEPENED

FOOTINGS, UNCOVERED AND UNEXPECTED UTILITY LINES, ETC.

13. CONSTRUCTION MATERIALS SHALL BE SPREAD OUT IF PLACED ON STRUCTURAL

FRAME SUCH THAT THE LOADING DOES NOT EXCEED THE DESIGN LIVE LOADS.

PROVIDE SHORING AND BRACING WHERE DESIGN STRENGTH HAS NOT BEEN

ATTAINED OR STRUCTURE IS NOT COMPLETE.

14. THE CONTRACTOR SHALL DETERMINE THE LOCATION OF UTILITY SERVICES IN

AREAS TO BE EXCAVATED BEFORE BEGINNING EXCAVATION. EXERCISE CAUTION

IN EXCAVATING AND TRENCHING.

15. THE STRUCTURAL ENGINEER SHALL NOT HAVE CONTROL OR CHARGE OF, AND

SHALL NOT BE RESPONSIBLE FOR, CONSTRUCTION MEANS, METHODS,

TECHNIQUES, SEQUENCES OR PROCEDURES, FOR SAFETY PRECAUTIONS AND

PROGRAMS IN CONNECTION WITH THE WORK, FOR THE ACT AND OMISSIONS OF

THE CONTRACTOR, SUB-CONTRACTORS OR ANY OTHER PERSONS PERFORMING

ANY OF THE WORK, OR FOR THE FAILURE OF ANY OF THEM TO CARRY OUT THE

WORK IN ACCORDANCE WITH THE CONTRACT DOCUMENTS.

16. VERIFY ALL OPENING DIMENSIONS THROUGH FLOOR, ROOF, AND WALLS WITH

MECHANICAL AND ELECTRICAL CONTRACTORS.

17. STRUCTURAL ELEMENTS ARE CENTERED ON GRID LINES AND GRID LINE

INTERSECTIONS, UNLESS DIMENSIONED OTHERWISE.

18. NOTIFY ARCHITECT OF ANY CONDITIONS NOT CONSTRUCTED PER THE CONTRACT

DOCUMENTS PRIOR TO PROCEEDING WITH CORRECTIVE WORK. SUBMIT

PROPOSED REPAIR TO THE ARCHITECT FOR ACCEPTANCE.

19. NOTHING SHOWN ON THE STRUCTURAL DRAWINGS SHALL BE CONSTRUED AS

ELIMINATING THE NEED FOR THE CONTRACTOR TO COMPLY WITH ALL OSHA

REQUIREMENTS. WHERE THE STRUCTURAL DRAWINGS APPEAR TO CONFLICT

WITH OSHA REQUIREMENTS, THE STRUCTURAL DRAWINGS REPRESENT FINAL

CONDITIONS ONLY.

20. THE CONTRACT DOCUMENTS HAVE BEEN PREPARED USING AVAILABLE DRAWINGS

AND SITE OBSERVATION AS PERMITTED BY ACCESS RESTRICTIONS DURING

DESIGN. DURING CONSTRUCTION, THE CONTRACTOR MAY ENCOUNTER EXISTING

CONDITIONS WHICH ARE NOT KNOWN OR ARE AT VARIANCE WITH PROJECT

DOCUMENTATION. CONTRACTOR SHALL NOTIFY THE ARCHITECT OF ALL

CONDITIONS NOT PER THE CONTRACT DOCUMENTS.

21. CONTRACTOR SHALL FIELD VERIFY ALL EXISTING STRUCTURAL CONDITIONS PRIOR

TO SUBMITTING SHOP DRAWINGS.

a. CONTRACTOR SHALL MAKE ALLOWANCE FOR THE RESOLUTION OF SUCH

DISCOVERIES IN THE CONSTRUCTION SCHEDULE.

22. SUBMIT A DIMENSIONED DRAWING OF ALL NEW OPENINGS THROUGH EXISTING

STRUCTURE AND SECURE APPROVAL PRIOR TO CUTTING. NEW OPENINGS MAY BE

EITHER SHOWN ON THE CONTRACT DOCUMENTS OR PROPOSED BY THE

CONTRACTOR. DRAWING SHALL SHOW:

a. VERTICAL & HORIZONTAL LOCATION AND SIZE OF NEW OPENING(S)

b. ALL EXISTING OPENINGS IN THE VICINITY OF THE NEW OPENING(S)

c. ALL EXISTING STRUCTURE (BEAMS, COLUMNS, SLABS, WALLS, ETC) IN THE

VICINITY OF THE NEW OPENING(S)

d. ALL REINFORCING BAR SIZES AND POSITIONS (LAYOUT LOCATION AND

DEPTH) CONFLICTING WITH OR IN THE VICINITY OF THE NEW OPENING(S).

23. SEE ARCHITECTURAL PLANS FOR INTERIOR PARTITIONS. PARTITION FRAMING

SHALL BE CONNECTED TO THE PRIMARY STRUCTURE IN SUCH A WAY SO AS TO

ALLOW FOR VERTICAL LIVE LOAD DEFLECTIONS OF 1/2” AT FLOOR FRAMING DO

NOT MAKE RIGID VERTICAL AND HORIZONTAL CONNECTIONS TO THE PRIMARY

STRUCTURE IN THE PLANE OF THE PARTITION.

SUBMITTAL NOTES

1. SEE THE PROJECT SPECIFICATIONS FOR DETAILED REQUIREMENTS.

2. SUBMITTALS SHALL NOT CONTAIN SUBSTITUTION REQUESTS WITHOUT PRIOR

AUTHORIZATION. SUBSTITUTION REQUESTS ARE FOR CONTRACTOR'S

CONVENIENCE. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL CHANGES

NECESSARY IF THEY CHOOSE TO SUBSTITUTE AND SHALL COORDINATE ALL

DETAILS. COST OF ADDITIONAL FIELD AND OFFICE WORK NECESSITATED BY

REQUESTS BY THE CONTRACTOR FOR A SUBSTITUTION REQUEST OR DUE TO

3. DELEGATED DESIGN SUBMITTALS:

DETAIL DELEGATED DESIGN ITEMS TO MEET THE PERFORMANCE AND

DESIGN CRITERIA ESTABLISHED AS PART OF THE BASE BUILDING

STRUCTURE. DELEGATED DESIGN ITEMS INCLUDE:

1. SHORING DESIGN

2. GALVANIC ANODES.

b. CONNECTION OF DEFERRED SUBMITTAL ITEMS TO PRIMARY STRUCTURE

BY DEFERRED SUBMITTAL SUPPLIER. DEFERRED SUBMITTAL SUPPLIER TO

PROVIDE CONNECTIONS AND FRAMING ARRANGEMENT TO AVOID LOADING

WHICH EXCEEDS THE CAPACITY OF THE ELEMENT BEING ATTACHED TO.

c. ALL DEFERRED SUBMITTALS TO BE ATTACHED TO PRIMARY STRUCTURE

WITH A PINNED CONNECTION. MOMENT CONNECTIONS TO PRIMARY

STRUCTURE NOT PERMITTED UNLESS NOTED ON DRAWINGS OR

APPROVED BY ENGINEER IN WRITING PRIOR TO SUBMITTAL OF DRAWINGS

OR CALCULATIONS.

d. LOADING AND LOCATION FOR ATTACHMENT OF DEFERRED SUBMITTAL

ITEMS ARE NOTED ON DRAWINGS AND ARE NOT TO BE RE-LOCATED OR

INCREASED WITHOUT WRITTEN APPROVAL.

e. WALLS, GRADE BEAMS AND THE UNDERSIDE OF CONCRETE ON METAL

DECK SHALL BE CONSIDERED CRACKED FOR THE PURPOSE OF DESIGNING

ANCHORS FOR ATTACHMENT OF DEFERRED SUBMITTAL ITEMS.

f. POWDER ACTUATED FASTENERS (PAF) INTO CONCRETE OR CMU SHALL

NOT BE USED TO RESIST TENSION LOADS. PAF SHALL NOT BE USED TO

RESIST GRAVITY LOADS WHICH INCLUDE BRICK VENEER.

REINFORCING STEEL NOTES

1. ALL REINFORCING STEEL SHALL BE DETAILED AND PLACED IN ACCORDANCE WITH

ACI 318 AND THE CRSI "MANUAL OF STANDARD PRACTICE" AS MODIFIED BY THE

PROJECT DRAWINGS AND SPECIFICATIONS.

2. DEFORMED REINFORCING BARS SHALL CONFORM TO THE REQUIREMENTS OF

ASTM A615 GRADE 60.

3. WELDING OF REINFORCING SHALL BE WITH LOW HYDROGEN ELECTRODES IN

CONFORMANCE WITH AWS D1.4. WELDING OF REINFORCING STEEL IS LIMITED TO

A706 REBAR.

4. NO REINFORCING BAR FIELD BENDS SHALL BE MADE UNLESS A706 REBAR.

5. WELDED WIRE FABRIC SHALL CONFORM TO ASTM A1064 GR. 65.

6. MINIMUM LAP OF WELDED WIRE FABRIC SHALL BE 6” OR TWO MESH PANELS,

WHICHEVER IS GREATER.

7. SPLICES SHALL BE MADE WHERE INDICATED ON THE STRUCTURAL DRAWINGS. IF

NOT INDICATED, SPLICE BEAM BOTTOM BARS OVER SUPPORTS AND TOP BARS AT

MIDSPAN. MAKE BAR CONTINUOUS AROUND CORNERS UON. ALL SPLICES TO BE

CONTACT TENSION SPLICE (CLASS B) UON.

8. DOWELS BETWEEN FOOTINGS AND WALLS OR COLUMNS SHALL BE THE SAME

GRADE, SIZE, SPACING, AND NUMBER AS THE SPECIFIED VERTICAL REINFORCING,

UON.

9. WHERE A 90°, 135°, OR 180° HOOK IS GRAPHICALLY INDICATED, PROVIDE

CORRESPONDING ACI STANDARD HOOKS UON.

10. MECHANICAL SPLICE COUPLERS: INSTALL IN ACCORDANCE WITH

MANUFACTURER'S INSTRUCTIONS

CONCRETE NOTES

1. ALL PHASES OF WORK PERTAINING TO THE CONCRETE CONSTRUCTION SHALL

CONFORM TO ACI 318 AND ACI 301, LATEST EDITIONS, WITH MODIFICATIONS AS

NOTED ON THE DESIGN DRAWINGS OR SPECIFICATIONS.

2. CONCRETE MIXES SHALL BE SUBMITTED FOR REVIEW, DESIGNED BY A QUALIFIED

TESTING LABORATORY AND SHALL BEAR THE SEAL OF AN ENGINEER LICENSED IN

THE UNITED STATES.
