Certificate No. REG20210001 is assigned to Overton Brooks VA Medical Center, owner identification number 11204, AI number 9007.

5.1.1 Required Every 30 Days.

5.1.2 Visually check spill prevention equipment for damage.

5.1.3 Remove liquid or debris from spill buckets.

5.1.4 Check for and remove obstructions in fill pipe.

5.1.5 Check fill cap to ensure it is securely on fill pipe.

5.1.6 For the interstitial monitoring, check for a leak in the interstitial area.

5.1.7 Check release detection equipment (Veeder-Root) to ensure it is operating with no alarms or unusual operating conditions present.

5.1.8 Manually stick gauge each tank, using water finding paste to determine water present and level.

5.1.9 Fill and monitoring ports: Inspect all fill or monitoring ports and other access points to make sure that the covers and caps are tightly sealed and locked.

5.1.10 Review and keep current release detection records.

5.2.10 Dispenser hoses, nozzles, and breakaways: Inspect for loose fittings, deterioration, obvious signs of leaks, and improper functioning.

5.2.11 Annual Fuel Quality testing.

5.3 Pre-clean inspection and Annual quality Sampling:

5.3.1 The contractor should take a fuel sample from multiple spots in each tank.

These samples should be sent to a third-party lab for analytics. Depending on the results of tank sample testing, cleaning may be required.

5.3.2 Tank cleaning should be accomplished by pumping out concentrated amounts of contaminated fuel, sludge, microorganisms, and water followed by filtration and adding chemicals.

6. Safety Precautions:

6.1. Any work performed should be done by trained and qualified professionals.

Mandatory safety meetings will be required before work can begin on any testing or cleaning process. Work will be done without interruption to normal business at the site. Ingress and egress into the hospital cannot be blocked by any equipment. Any fire hazards should be eliminated at this time considering the contents of the tanks.

Fire extinguishers will be always required at the work area. Spill containment such as absorbing pads and booms should be on hand and readily available. Pre-work equipment inspections should be completed to avoid any spillage from damaged hoses, pumps, etc.

7. Cleaning Process:

7.1. Initially, tanks will be scanned and concentrated areas of sludge, water, or dirty fuel should be removed by pumping these areas. Cleaning chemicals and additional filtering will be introduced. Filters will continue to be changed out to lower micron as the tanks and fuel is cleaned. Chemicals should kill any microorganisms or algae growth inside the tanks and remove any sludge buildup on tanks walls or floor.

7.2. All filters and chemicals will be provided by the contractor.

8. Clean Sample and Tank Closure:

8.1. A sample should then be taken of the fuel after fuel and tanks are cleaned. This sample will also be sent to an outside lab for results. Extra chemicals should be added to the tank to help prevent growth in the tank over time. Contractor will then close tanks using proper seals, gaskets, etc. to ensure no new contaminates enter the tanks around the openings.

9. Tank Decommission:

9.1 The LDEQ oversees the closure process, which varies based on whether the tank is aboveground or underground and whether it contained hazardous materials. For USTs, closure requirements under LAC 33:XI mandate that owners notify the LDEQ at least 30 days before decommissioning. Tanks must be emptied, cleaned, and either removed or filled with an inert material. If contamination is suspected, a site assessment is required, and corrective action may be necessary before closure is approved.

9.2 The UST for unleaded fuels, 1,000 gallons, must be decommissioned according to

LDEQ requirements, fueling point and bollards removed, area made safe. UST must remain in ground due to location next to the Boiler Plant foundation. Unleaded fuel will be polished/water removed and placed into existing 250-gallon AST (tote). Any additional fuel remaining must be disposed of as hazardous waste or recycled and those funds returned to the VA FCP 900 recycling fund.

10. Disposal of Waste:

10.1. All materials and waste removed from the tanks and surrounding areas will be disposed of in compliance with all current environmental laws; local, state, and federal, regulations and notices.

11. Documentation/Reports:

11.1 Documentation shall include detailed descriptions of the scheduled and unscheduled maintenance procedures performed, including replaced parts and prices required to maintain the equipment in accordance with manufacturer specifications.
