Statement of Work

Building 4 Replace Transformer Project No. 667-18-110

SCOPE OF WORK (SOW):

Overton Brooks VAMC has a requirement to correct electrical discrepancies, Building 4, Engineering Service, Overton Brooks VAMC, Shreveport, LA. Contractor shall provide all labor, materials, and equipment necessary.

Construction will include, but not limited to, the replacement of the transformer, upgrading deficient lighting, relocating and upgrading electrical panels, ensuring adequate system ventilation and cooling is provided in Closet 4-4, constructing two rooms (Communications and Electrical) to house panelboards etc. and replacing of the automatic transfer switches to include bypass isolation. This contract will also identify all panel circuits and relocate panels to provide for required clearances for safe electrical work and maintenance. Construction shall correct the electrical discrepancies noted in the 2015 Facility Condition Assessment for Building 4 (Attached).

The construction shall be in accordance with (IAW) the latest version of all applicable VA Design Guidelines and Life Safety Code. Continued function of Building 4 during normal work hours is required.

The C&A requirements do not apply, and that a Security Accreditation Package is not required

PERIOD OF PERFORMANCE:

The Contractor shall complete the work 180 Days from the issuance of the Notice to Proceed.

CONTRACTOR SITE VISIT AND FIELD VERIFICATION PRIOR TO BIDDING:

Contractor shall be responsible for attending the Pre-Bid Site visit prior to bidding to verify all existing conditions and take any necessary measurements required to adequately bid the project. If the contractor chooses not to attend the Pre-Bid site visit, the contractor may submit their bid at their own risk.

INFECTION CONTROL DURING CONSTRUCTION:

Contractor shall follow and comply with the established Infection Control Risk Assessment (ICRA) specific for the project. Contractor is responsible for dust control during demolition and during hauling operations.

DEMOLITION:

Contractor shall be responsible for locating and marking all underground utilities prior to demolition. Contractor is responsible for ensuring enough new base course material is installed to establish required grade and slope.

Contractor shall protect all adjacent areas of the hospital, especially windows and doors during all operations.

Repairing any damage shall be the sole responsibility of the contractor. If any damage is noticed prior to starting work that may be misconstrued as damage caused by the contractor, the project manager should immediately be notified, and the damage should be documented as existing prior to beginning demolition.

NEW WORK:

New work includes, but is not limited to:

1. Replacing the transformer.

2. Upgrading deficient lighting.

3. Relocating and upgrading electrical panels.

4. Ensuring adequate system ventilation and cooling is provided in new rooms.

5. Constructing two rooms (Communications and Electrical) to house panelboards etc.

6. Replacing the automatic transfer switches to include bypass isolation.

PHASING, NOISE, SMELLS, AND AFTER-HOURS WORK:

The area to be renovated will be occupied during construction, therefore, adequate scheduling will need to be coordinated with the COR. Extensive noise or smell related work may be required to be performed after hours (at night or on weekends). Any required outages needed to perform the work, will be scheduled at the convenience of the hospital, and may also require after hours work (at night or on weekends).

DELIVERIES:

Contractor shall coordinate delivery, storage, and handling requirements with the VA COR to designated prepared areas to minimize site storage time, impact to the hospital, and potential damage to stored materials.

FINAL CLEAN, PRE-FINAL & FINAL INSPECTIONS:

Once all work has been completed, the Contractor shall adequately clean the project site and request a Pre-Final Inspection. Upon completion of the Pre-Final punch list items, the contractor shall request a Final Inspection.

Each of these inspections will be scheduled at the convenience of the hospital.

SUBMITTALS:

Provide 3 hard copies & email an electronic copy (including transmittal sheets for each) to the COR.

Contractor shall provide submittals as detailed in the specifications and samples of all finishes showing the full range of available colors and options to be selected by the VA-ID and COR. Required submittals include but are not limited to:

1. Project Schedule

2. Project Schedule of Values

3. Infection Control Plan

4. Submittal Register (list all submittals required for each specification)

Overton Brooks VA Medical Center, 510 E. Stoner Ave, Shreveport, LA 71101

Additional Guidance:

• Contractor shall coordinate all material and equipment deliveries with the COR.

• Contractor shall remove and dispose of waste daily in accordance with all legal and VA requirements.

• Contractor is responsible for backfilling and restoring over excavated areas to existing conditions and like materials up to new work.
