Description of Interim Life Safety Measures (ILSM) Used

Project #: _667-15-104____ Title: Renovate Cardiac Catherization Lab Date: April 28, 2021

Based on the responses to the evaluation questions, provide a description of the ILSM that will be implemented during the project to compensate for the deficiency or condition. Attach additional sheets if necessary.

-If the contractor has to block an exit, they will hang signage at each location on the floor that directs traffic towards an exit, using a sign stating, “No Exit Due to Construction,” adding directional signage to the next nearest exit, and covering corridor exit signs. The contractor will remove this signage after removing temporary barriers. At no time, will stairwell egress be blocked, without prior notice to COR and Safety Department.

-A corridor width of 44” shall be maintained at all times for patient care and transport.

-When generating dust, the contractor will tape and bag the smoke detectors within the area under construction. The tape and bags will be removed when workers are not present during construction, ie. at night. The boiler plant operator will deactivate and reactivate the smoke detection system to avoid unnecessary alarms as required.

-Fire sprinklers will be turned up and the heads replaced with correct upright pendant heads. Fire system will be de-energized and re-energized each day on the floor during the time the sprinkler heads and pendants are being changed. These outages will be coordinated through the Safety Department.

-Any Life Safety fire detection/alarm section out of service for greater than 4 hours in a 10-hour period, will require a fire watch.

All asbestos and hazardous materials remediation will be covered under a separate ILSM.

Signatures required as indicated below. Completed form will be maintained in the project file.
