About this file

This document is an Amendment of Solicitation/Modification of Contract (Standard Form 30) issued by the Department of Veterans Affairs Network Contracting Office (NCO) 15 in Leavenworth, Kansas for RFQ 36C25526Q0384 regarding Medical Gas Maintenance Service and Inspection for the Poplar Bluff VAMC. The amendment extends the receipt deadline for offers to July 10, 2026 at 5:00 PM CDT and provides answers to four questions submitted by offerors. The contracting officer is Laura Ferguson.

The Questions and Answers (Addenda #1) address specific equipment inventories and service scope clarifications. Answer #1 provides a comprehensive equipment count at two locations: John J Pershing VAMC Poplar Bluff, Missouri includes 1 oxygen bulk system, 5 total medical air skids, 1 medical equipment air scroll, 2 vacuum skids, 2 master alarm panels, 21 zone valves, 21 area alarms, and patient outlet terminals (122 oxygen, 117 medical air, 116 vacuum, 3 medical equipment air dental, and 4 booms at Cape HCC). Cape Girardeau HCC includes 1 oxygen bulk, 1 medical air scroll, 1 vacuum skid, 2 master alarm panels, and patient outlet terminals (17 oxygen, 14 vacuum, 13 medical air, and 4 WAGDs). Answer #2 confirms 4 booms at Cape HCC. Answer #3 indicates a previous annual inspection report is attached. Answer #4 clarifies that annual inspections are required at both locations, but preventative maintenance is required only at the Poplar Bluff facility, as Cape HCC is a lessor-owned building.
